SECTION A - FBO NOTICE A.1COMBINED SYNOPSIS/SOLICITATION Under FAR Subpart 12.6 - Streamlined Procedures for Evaluation and Solicitation for Commercial Items, the Department of Veterans Affairs, Great Lakes Acquisition Center, Southern Tier, is providing this combined synopsis/solicitation for commercial items. This notice constitutes the only solicitation that is being issued as a request for quotation (RFQ); and a written solicitation will not be issued. The RFQ# is VA69D-15-Q-1288 for providing the services of on-site diagnostic medical physics support for the Radiology Equipment located at CAPT James A. Lovell Federal Health Care Center, North Chicago, IL. This acquisition is being solicited as a 100% Small Business set-aside, using NAICS Code 541690 (Other Scientific and Technical Consulting) with a size standard of $15M; and Product Code H965 (Inspection of Medical, Dental Equipment). The contract will be a firm fixed price. Persons may identify their interest and capability to respond to this requirement by furnishing the information/documentation listed in the attached description document by September 16, 2015, 12:00PM CST.
