- This is a complete copy of RFQ N00183-15-T-1328. Please See, Read, & Adhere to Section C RFQ Information before quoting on this RFQ. Also, this RFQ will close on MONDAY, 14 SEPTEMBER 2015. If there are any questions, please contact Carolyn Johnson (phone: 757-953-5740; email: adline.c.johnson.civ@mail.mil). This solicitation is restricted (set a-side) to Small Business. Requirement to be awarded on a brand name or equal basis. Technical evaluation of vendor's quote will consist of technical capability to provide and perform product specifications as stated in Section B & price. The item on this RFQ is the Recorder System for attended sleep studies manufactured by Compumedics USA, Inc. for the Sleep Laboratory at Naval Medical Center, Portsmouth, VA. Quotes should reflect pricing on all items for consideration. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). If freight charges apply, indicate if charges will be "per shipment" or "single one-time charge". Contractor shall supply a current published price list or if material is listed on a GSA/VA Federal Supply Schedule contract, please submit contract number, expiration date, SIN, & MOL for supplies &/or services being procured along with the Published Price List. Refrigerated, frozen, or hazardous material must be indicated as such & a copy of the MSDS (Material Safety Data Sheet) must be supplied for any hazardous material. Specify any special handling instructions. Solicitation closes on MONDAY, 14 SEPTEMBER 2015.
