- This is a Sources Sought Notice (Request for Information). The Government intends to establish Blanket Purchase Agreements (BPAs) for Blood Donor Specimen Testing Services as defined in the attached Performance Work Statement (PWS) for the Department of Pathology at Tripler Army Medical Center, Hawaii. Calls will be placed against the BPA as services are required. Period of performance will be for a one-year base period. NAICS code is 621991. Interested parties shall reply to Regional Health Contracting Office - Pacific VIA EMAIL to lee.h.miyashiro.civ@mail.mil. TELEPHONE REQUESTS WILL NOT BE HONORED. Please indicate your interest and/or ability to provide the required services and a current copy of your price list NO LATER THAN 11 SEPTEMBER 2015, 10:00 AM Hawaii Standard Time or 4:00 PM Eastern Time. VENDOR MUST BE ABLE TO PERFORM AND POSSESS ALL SERVICES AND LICENSES/CERTIFICATIONS/ACCREDITATIONS as defined in the PWS to be considered for this BPA.
