- The purpose of this Amendment 0005 is to upload a Revised Combined Synopsis Solicitation document which removed the requirement for Proof of Insurance with the submission of quotations and extends the submission of quotation due date from 04 September 2015 to 10 September 2015, reference the Track Changes identified on pages 1, 4, 5, and 12 of the Revised Combined Synopsis Solicitation document. The purpose of this Amendment 0004 is to upload the Questions and Answers document that was inadvertently left off from posting of Amendment 0003. The purpose of Amendment 0003 is to post Questions and Answers regarding the Combined Synopsis Solicitation, W911RZ-15-T-0149. All questions or inquiries concerning this solicitation shall be submitted in writing via email to allen.b.chandler.civ@mail.mil no later than 8:00 A.M. MDT on 31 August 2015. The purpose of Amendment 0002 to the Combined Synopsis Solicitation W911RZ-15-T-0149 is to replace PWS Attachment 6 Facilities List for Fort Carson & PMCS and PCMS Map. The revisions include: (1) add an additional build to the PCMS Facilities List, (2) changed the PCMS Facility No's to align with the PCMS Map, and (3) identified the calculation of the total square footage, per service type and per location. See Rev 1 PWS Attachment 6 with yellow highlights for changes. All other terms of the Solicitation and documentation remain unchanged. The purpose of Amendment 0001 to the Combined Synopsis Solicitation W911RZ-15-T-0149 is to add the Combined Synopsis Solicitation document W911RZ-15-T-0149 for Pest Management, the PWS Exhibit 1 Contract Reporting Requirements List, PWS Exhibit 2 Estimated Workload, WDOL Wage Determination #05-2079 Revision 17 Dated 7/08/2015 El Paso County, and WDOL Wage Determination #05-2083 Revision 18 Dated 7/08/2015 Las Animas County. All other terms of the Solicitation and documentation remain unchanged. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
