- Attachment 1-Drawings titled Radio Towers & Cabinet Steel Base-Rio Grande National Forest, Sheet S1 Form SF-1449 and solicitation pkg. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number AG-82AK-S-15-0023 is being issued as a Request for Quotes. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. This project is 100% set aside for small business concerns. The North American Industry Classification System (NAICS) Code for this project is 332312 Fabricated Structural Metal Manufacturing. The small business size standard is 500 employees or less. The Rio Grande National Forest requires the fabrication of several structural steel assemblies to function as the base for mounting radio communications equipment and pre-manufactured towers at remote communication sites across the forest. This project includes structural steel fabrication, finishing and delivery of the finished assembly to Monte Vista, CO. Contract time is 30 calendar days from award date of the contract. The solicitation for this project will be available only through the Federal Business Opportunities website www.fbo.gov. All questions pertaining to this solicitation must be submitted to the address provided in the solicitation or by email message to Kim Decker at kdecker@fs.fed.us no later than five (5) days preceding the ending date of responses to this solicitation. All Offerors shall be registered in the System for Award Management (SAM) at www.sam.gov in accordance with Federal Acquisition Regulations (FAR) 52.204-7 and must respond to all evaluation criteria in Section "L" and "M" of the solicitation to be considered responsive. Offerors that fail to meet these selection criteria will not be considered in the Technical Evaluation of offers. The solicitation is attached to this announcement. The actual due date is identified within the solicitation. The following FAR Provisions apply to this acquirement: Number 52.212-2, titled Evaluation - Commercial Items; and Number 52.212-3, titled Offeror Representations and Certifications - Commercial Items. The following FAR Clauses apply to this acquirement: Number 52.212-4, titled Contract Terms and Conditions - Commercial Items; and Number 52.212-5, titled Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. Additional applicable clauses can be found in the solicitation package. U. S. Department of Agriculture Acquisition Regulation (AGAR) clause number 452.209-71, titled Assurance Regarding Felony Conviction or Tax Delinquent Status for Corporate Applicants - Alternate 1 (MAR 2012) and AGAR provision number 452.209-70, titled Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction (DEVIATION 2012-01) (FEB 2012) Alternate 1 (FEB 2012), apply to this acquisition. Offerors shall submit their quote, in the following format: The following documents must be submitted as four (4) separate attachments and/or documents by the offeror or respondent for evaluation to be considered responsive: 1.Completed, Signed Offer. (1)Complete blocks 12, 17a, 30a, 30b, and 30c on the SF 1449. (2)If amendment(s) are issued to the base solicitation document(s), please acknowledge any and all amendment(s) by signing and returning form SF-30. 2.Price Provision-Section B, Schedule of Items (page 4) (1)Include unit pricing and totals for all pay items. Do not deviate or alter the Section B blocks. 3.Business Provision. (1)Within Section F, Deliveries or Performance, Complete FAR 52.211-9, Desired and Required Time of Delivery, and Complete Offeror's Proposed Delivery Schedule, page 6. (2) Within Section K, Representations, Certifications, and Other Statements of Offerors, complete as appropriate, and submit Section "K" Clauses FAR 52.212-3, and AGAR 452.209-70. Note that both Clauses must be submitted if prepared or if not completed (see pages 16-29). 4. Technical Provision. (1)Submit documents as stated within Section L.2, paragraphs A, B, and C. Submit quotation package no later than SEPTEMBER 8, 2015, 2:00 pm, MDT Email submission of proposals is preferred and acceptable as long as all required information is submitted, including required signatures.
