- Attachment 2: Statement of Objective Attachment 1: Clauses & Provisions Animal Feed Combined Synopsis/Solicitation IAW FAR 12.603(c)(2) (i) Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Department of Homeland Security (US DHS) has a requirement and requesting firm fixed price type quotations for the supply and delivery of animal feed for the Department of Homeland Security (DHS), Science & Technology (S&T), Plum Island Animal Disease Center, located in New York; (ii) The solicitation number is HSHQPD-15-Q-00018 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76. (iv) There are no set-aside requirements. The NAICS code for this solicitation is 453998 (All Other Miscellaneous Store Retailers) and the Small Business Size Standard is 500. (v) List of contract line item number(s) and items, quantities and units of measure: Brand Name or equal to the salient characteristics of: 0001 - Swine Feed #1 : 50 lb. Bag of Porcine Grower # 5084Qty: 1,520/Year 0002 - Donor Swine Feed #2: 50 lb. Bag of Purina MiniPig Grower #5081Qty: 820/Year 0003 - Cattle Feed #1: 50 lb. Bag of Alfalfa Cubes #1463Qty: 5,680/Year 0004 - Cattle Feed #2: 50 lb. Bag of Rumilab Maintenance #5510Qty: 2,450/Year (vi) Description of requirements for the items to be acquired: The vendor shall supply and deliver the aforementioned Brand Name or Equal to items (to include salient characteristics). • 50lb bags with expiration date per bag shall be no later than 100 days from delivery. • Dehydrated alfalfa meeting or exceeding; Protein 13.5%, Fat 1.5%, Fiber 25.5%, and 9% Ash • Items shall be delivered in the amount that is requested monthly. The amounts requested shall not exceed the amounts in the quantity column for the Delivery/Price Schedule (i.e. NTE 50 Bags). vii) Date(s) and place(s) of delivery and acceptance and FOB point: Delivery shall be F.O.B. Destination to: Dept. of Homeland Security Plum Island Animal Disease Center Plum Island Warehouse 40550 Route 25 Orient Point, NY 11957 Attn: Philip Doucett DHS MS10, ARU, Building 101 (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) The resultant purchase order will be awarded based on Lowest Price Technically Acceptable (LPTA) using simplified acquisition procedures in accordance with FAR 13.5. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (xi) The Clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii) No other additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements) have been determined to be necessary for this acquisition. (xii) The Defense Priorities and Allocations System (DPAS) and assigned rating is None. (xiv) The date, time and place offers are due. All responsible sources may submit a quotation to kimberly.oneill@st.dhs.gov, which if timely received, shall be considered by the Agency. Companies must have valid DUNS numbers and be registered with Central Contractor Registration (CCR) and shall provide the company Tax Information Number (TIN) with their offer. The closing date and time for receipt of quote is Tuesday, September 8th 2015 at 11:00 a.m., Eastern Standard Time.
