- This is a Small Business Set Aside Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITAITON WILL NOT BE ISSED QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award on a competitive basis, using Simplified Acquisition Procedures, a firm fixed priced award for the 'Regional Office LaserFiche Document Imaging/Management Systems' The Statement of Work and Technical Evaluation Criteria are attached. Responses shall be no more than 10 pages and shall include the following: 1) Technical Understanding and Approach, and 2) Price - fully loaded labor rate and anticipated hours. The award will be made based on the Lowest Price that is Technically Acceptable meeting or exceeding the acceptability standards for non-cost factors. All responsible sources must be registered in the System for Award Management (SAM) at www.sam.gov. The period of performance is September 15, 2015 - September 14, 2016. The North American Industry Classification Code (NAICS) is 423430. Responses must be sent electronically in MS Word to renee.wallaceabney@cms.hhs.gov no later than 11:00 am Eastern Standard Time, (EST) on or before, Friday, September 14, 2015. ALL QUESTIONS SHOULD BE SUBMITTED ELECTRONICALLY BY September 4, 2015. QUESTIONS WILL NOT BE ANSWERED BY TELEPHONE. All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated
