Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This Request for Quote (RFQ) is in response to the requirements of the United States Army Medical Research Acquisition Activity (USAMRAA) on behalf of the Armed Forces Medical Examiner System (AFMES), The Office of the Armed Forces Medical Examiner's (OAFME) to provide Picture Archiving and Communications System (PACS) Upgrade and Data Migration according to specification listed in the Request for Quote. The technical documentation for offerors proposals of equal models must meet the minimum essential characteristics described as attached. Equal modes are required to provide sufficient descriptive literature in accordance with FAR Clause 52.211-6, Brand Name or Equal, to allow for through evaluation of equal proposals. This requirement will be a Firm Fixed Price (FFP) contract. Performance requirements for this contract are provided in the attached Minimum Essential Characteristics (MECs), and Performance Work Statement (PWS) of this RFQ. Your submission must include your quote as a Firm-Fixed Price order and shall also address all requirements as stated in the request for quote. The Offeror's price proposal shall also include all other costs and reductions, discounts, or rebates offerred. Offeror's quotes shall be submitted in English and in US Dollars. It shall also remin open for a minimum of 90 days from the date of submission. Offerors must certify and return the representations and certifications in 52.212-3 and 252.209-7999. The Government intends to award an order without discussions. Offeror shall provide their best, complete proposal upon initial submission. The Government reserves the right to hold discussions if deemed necessary by the Contracting Officer. PROPOSAL DUE DATE: No Late Than (NLT): ): 08 September 2015 at 1:00 PM Eastern Time. Price quotes must be emailed to Kimberly Hannan at kimberly.s.hannan.civ@mail.mil The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83-2-2, dated August 4, 2015. There are no set-aside restrictions, the associated NAICS code 334510 and the small business size standard 500 employees. FAR Clause 52.212-1, Instructions to Offerors Commercial Items, applies to this acquisition. In addition, FAR Clause at 52.212-2, Evaluation Criteria Commercial Items also applies. You must include a completed copy of the provision at FAR Clause 52.212-3, Alt I Offer Representation and Certifications-Commercial Items, with your offer. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items, and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, apply to this acquisition. Response to this notice shall contain sufficient information to establish the interested parties' bona-fide capabilities for fulfilling the requirement and include: unit price, list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination and Origin), the Dun & Bradstreet Number (DUNS), and the Taxpayer Identification Number (TIN).
