- Carville Job Corps Center Vendor Self Certification Form Driver Education Instructor Dynamic Educational Systems Inc. (DESI), contracting for the U.S. Department of Labor for the operation of the Carville Job Corps Center, a residential adolescent training facility located at 5465 Point Clair Road, Carville, LA 70721, is currently soliciting written bids for Driver Education Instructor. All quotes must be submitted in writing to the Purchasing Department by September 28, 2015 by 9:30 a.m. Quotes received after the deadline and/or without all requested documentation will not be considered. Vendor must submit a written bid on company letterhead to include scope of work ID#CJCC2015-5 at the top each of page. Vendor must provide licenses for doing business in the State of Louisiana, all employee licenses required for performing work in the state of Louisiana, certificate of liability insurance and list of references if no past experience with Carville Job Corps Center. The period of performance is November 1, 2015 thru October 31, 2016.
