Description
Background: The Lebanon VA Medical Center (LVAMC) has a requirement for a bottled water service. We have some areas of the hospital with unpotable water, some areas with no drinking water available and a need to provide water for Chemo Patients with Compromised immunity.. Scope of Work: "DESCRIPTION OF SERVICE. The contractor shall provide all management, labor, materials, and transportation necessary to provide filtrated bottled water free standing dispensers/coolers and 5 gallon bottled water refills to the VA Medical Center (VAMC) of Lebanon, Pennsylvania (PA). Unless specified as "Government Furnished," the contractor shall provide all personnel, equipment, materials, transportation, quality control and supervision to perform this service. " Background: The purpose of this service is to provide potable quality drinking water to Veterans. " Performance Period: October 1, 2015 to September 30, 2016, with 4 option years. "GENERAL REQUIREMENTS 1 The contractor shall deliver bottled water refills and pick up empty water bottles at the Lebanon VA Medical Center every other week per attachment A schedule. 2. The contractor shall deliver bottled water to the CBOCs every month per required par levels identified in attachment A. 3. The Contractor shall ensure deliveries are made between 8:00 a.m. and 12:00 p.m. on Thursdays. A.SPECIFIC REQUIREMENTS 1. The contractor shall check in at Building 6, (Warehouse) 1700 South Lincoln Avenue Lebanon, PA 17042 at the Lebanon VAMC upon arrival and before delivery. 2. Dispensers. Contractor shall provide 25 cooler type bottled water (5 Gallon) dispensers that dispense cold water and at least 2 that provide cold and hot water if needed. Contractor is also to provide disposable drinking cups or cones to be replenished with water delivery, along with the appropriate dispenser attached to the cooler. Repair/replacement of contractor provided dispensers is at no cost to government. 3. Water. The contractor will deliver full bottles of water to designated delivery points without exceeding par levels in accordance with Water Delivery Sheet (Attachment A). After water delivery is completed, the contractor shall sign and provide a completed Attachment A to warehouse personnel. 4. The Lebanon VA owns 3 water coolers that will require water delivery in accordance with Attachment A. 5. The contractor shall place coolers/dispensers and maintain locations in accordance with Attachment A. The contractor shall provide labor to move coolers/dispensers if a location change is to occur upon direction of the Contracting Representative (COR) and will accomplished at the next scheduled delivery. B.EMERGENCY WATER AGREEMENT 1.Under this agreement, the contractor shall supply available bottled water needs in the event of a local or national declared emergency or natural disaster at the same price awarded upon demand and outside delivery terms agreed upon limited to Lebanon VAMC. C.DELIVERY 1.May include, but not exceed: a. 27 Dispensers/Coolers b. Lebanon VAMC: 80 bottles per delivery 1.2.Signing of Receipts. After delivery of the bottled water, the contractor and VA employee shall sign Attachment A, validating the total number of filled water bottles delivered. Attachment A will be kept on file with Logistics Service. 1.3."On Call" Delivery Service. Upon notification by the Contracting Representative or designee, the contractor shall deliver refilled bottles within one calendar day after notification in any area covered under this contract. The price for pickup and delivery services, to include "On Call" services, shall be included in the unit price for the bottled water refills D.GOVERNMENT FURNISHED PROPERTY AND SERVICES 1.The government shall provide, without cost to the contractor, electricity. The contractor shall use electricity from existing outlets. H. GENERAL INFORMATION 1.Hours of Operation. Services under this contract shall be performed during the hours of 8:00 a.m. through 12:00 p.m. on Thursdays. 2. Holidays. Normally the contractor is not required to provide services on the following days: New Years DayLabor Day Martin Luther King DayColumbus Day Presidents DayVeteran's Day Memorial DayThanksgiving Day Independence DayChristmas Day 3.All personnel operating a vehicle shall be a licensed driver. All vehicles shall be licensed and insured. 4. The contractor's employees shall be restricted to those areas necessary to perform work under this contract as well as direct routes to and from work site. Delivery: Delivery shall occur within 30 days of contract award. Delivery shall be made to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn: Warehouse. This solicitation is set-aside for Small Businesses. The NAICS Code for this project is 312115 and the Small Business Size Standard is 500 Employees. Any business that is not considered a small business will not be eligible for award. A solicitation document will be available electronically on or about August 28, 2015 at the following website: www.fbo.gov. The estimated Period of Performance is October 1, 2015 through September 30, 2016 with four 12-month option period that may be exercised at the discretion of the government. Bidders are advised that it is your responsibility to review and monitor the website frequently to ensure you have the most up to date information, including amendments.
