CONTINUATION PAGE STATEMENT OF WORK Plumbing Dead Leg Removal Project #595-14-177 1.The scope of work shall generally consist of, but not limited to the following; providing all labor, materials, tools, equipment, permits, testing, and reports required to remove all the plumbing dead legs in all areas of the Lebanon VA Medical Center, 1700 South Lincoln Ave., Lebanon, PA as defined by the drawings and specifications. The intent of this project is to replace the currently identified plumbing dead legs with the proper piping that eliminates the dead leg. Dead legs discovered during the construction process shall be reported to the COR who will then initiate the change request process that approves any additional work. The project will include de-construction, demolition, reconfiguration, asbestos abatement, ceilings, alarm systems, plumbing, pipes, and associated structural components as shown on drawings and specifications. 2.Work consists of mainly plumbing work but is not limited to dead leg removal. Also included will be cutting and repairing ceilings, insulation, and other associated work as shown on drawings and specifications. Any dead legs discovered during the construction process shall be identified to the COR in order to initiate a formal change request process that approves the additional work. 3.This project shall be bid as the price to complete all plumbing, asbestos abatement, insulation, and other work as identified in the drawings and specifications. All work shall be completed within 280 calendar days from receipt of Notice to Proceed. Due to multiple renovations and construction projects, the Lebanon VA Medical Center had numerous plumbing lines which contain dead legs. The stagnant water in these dead legs significantly increases the likelihood for the formation of Legionella Disease in the water supply in the Medical Center. The purpose of this project is to remove all plumbing dead legs throughout the facility. The architect engineering firm contracted to conduct a survey of the facility has identified 325 dead legs of pipe in buildings 1,3,4,5,17,18,19,22,23,24,100,102,The selected contractor will provide all labor, materials, tools, equipment, permits, testing, and reports required to remove all the plumbing dead legs in all areas of the Lebanon VA Medical Center, 1700 South Lincoln Ave., Lebanon, PA as defined by the drawings and specifications. The intent of this project is to replace the current identified plumbing dead legs with de-construction, demolition, reconfiguration, asbestos abatement, pipe insulation, ceiling openings, ceiling closings, alarm systems, plumbing, pipes, valves and associated structural components as shown on drawings and specifications. ? Plumbing Dead Leg Removal Pre-Bid Questions and Answers 1.ACM abatement is noted as being our responsibility. We can find no report or quantified recap of affected material(s) from which to figure in our bid. Can you please clarify this requirement? There is no identified asbestos identified on the dead legs to be removed on this project. The latest asbestos inspection will be made available to the successful bidder after contract award. 2.Can you please verify that we will be responsible for testing of the existing fire protection systems in each area of dead leg removal, in each room of all affected buildings (per Gen Requirements, section 010000, page 7)? No testing of fire protection systems with dead legs in affected areas will be required. 3.Where on the plans is a list of "Government Furnished Property" for us to install, as noted in General Requirements, section 010000, pages 22 and 23? There is no Government Furnished Property on this project. 4.Plumbing section 220511, page 5 mentions the need for our Plumber to create and submit Coordination Drawings. This seems like a difficult chore in that we are not fitting other mechanical and electrical equipment in a new "above-ceiling" type space. Frankly, we are not entirely sure as to how to even create such "drawings." Will this remain a requirement in the bid? If so, what is the desired outcome/submission? No coordination drawings will be required. The vendor must provide red line drawings to the A/E in order to complete as built drawings. 5.In comparing the Dead Leg Schedules with the related floor plans, we are happening upon a number of apparent glitches, including: oDrawing 9/58: Noted location 1-G-G006-001 does not appear on the schedule oDrawing 11/58: Noted location 1-1-104C-001 does not appear on the schedule oDrawing 17/58: Noted location 1-4-435-pipechase does not appear on the schedule oDrawing 19/58: Noted location 1-5-585-001 does not appear on the schedule oDrawing 27/58: Noted location 5-B-001 does not appear on the schedule oDrawing 37/58: Noted location 18-1-126-001 does not appear on the schedule oDrawing 38/58: Noted location 18-2-241-001 does not appear on the schedule oDrawing 39/58: Is noted location 18-3-314A-001 (pointing to Room 311) mis-marked? oDrawing 44/58: Noted location 19-2-256A-001 does not appear on the schedule oDrawing 44/58: There are two locations with the same mark (19-2-229-001) that point to different rooms oDrawing 5/58, Building Number 22, Room Number 1, Dead Leg Numbers 4 and 5 do not show up on drawing 46/58, Floor Plan oDrawing 5/58, Building Number 102, Room Number G006, Dead Leg Number 1 (both marks) do not show up on drawing 57/58, Floor Plan Drawing 9/58: Dead leg 1-G-G006-001 is located in the drawing and specifications, but it was not listed in the master. Drawing 11/58: Noted location 1-1-104C-001 is a mixing valve, not a dead leg. Drawing 17/58: Noted location 1-4-435-PIPECHASE is a mixing valve, not a dead leg. Drawing 19/58: Dead leg 1-5-585-001 is located in the drawing and specifications, but it was not listed in the master. Drawing 27/58: Dead leg 5-B-001 shall be add to contract documents, see attached. Drawing 37/58: Noted location 18-1-126-001 is a mixing valve, not a dead leg. Drawing 38/58: Dead leg 18-2-241-001 is located in the drawing and specifications, but it was not listed in the master. Drawing 39/58: Location 18-3-314A-001 should be label 18-3-311-001 Drawing 44/58: Dead leg 19-2-256A-001 is located in the drawing and specifications, but it was not listed in the master. Drawing 44/58: Location 19-2-229-001 should be labeled 19-2-229C-001, this is located in room 229C. Drawing 46/58: Dead legs 22-B-001-004 and 22-B-001-005 area located in building as 22-B-001-003. Dead leg descriptions of 22-B-001-004 and 22-B-001-005 area only located in the master schedule. Drawing 57/58: Dead leg 1-G-G006-001 is located on drawing 9/58. There is both domestic cold water and domestic hot water dead leg. Dead leg descriptions for 1-G-G006-001 are located in the project manual. ?
