- Vendor Self-Certification Form (Required) W-9 (Required) MTC Terms & Conditions DBA Wage Determination CBP This is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center. Tongue Point Job Corps Center is requesting proposals from plumbing, heating, and air-conditioning contractors. Details of the solicitation are in the attached Construction Bid Package (CBP: TPJCC Academic Building #3 Return Air 15-19.pdf.) Please accept this as your invitation to attend our pre-bid meeting for this project at TPJCC, located at 37573 Old Highway 30., Astoria, OR 97103. The meeting will be conducted on Friday, September 11, 2015 at 2:00pm (PST), with bids due on Thursday September 17, 2015 at 5:00pm (PST). Please see the attached Construction Bid Package (CBP) for details. Please pay close attention to page 2 of the Construction Bid Package Instructions to Bidder. In addition to the bid sheet, all valid bids must include 3 (three) references, DBA Worker Classification/Rates, a W-9, and completed MTC vendor self-certification form (attached). Bidders must self-certify under the NAICS code listed in the CBP. (NAICS codes can be found at http://www.census.gov/eos/www/naics/) This solicitation is listed under NAICS code: 238220: Plumbing, Heating, and Air-Conditioning Contractors. Note: A site-inspection will be required by the bidder or their appointed company representative for the bid to be considered valid. The best opportunity for this inspection will be during the pre-bid meeting at the time stated above.
