- The Government intends to procure commercial item services under (FAR Part 12 and FAR 13) for VA Greater Los Angeles Healthcare System. The NAICS code is 624229, Size Standard is $15 Million. The U.S. Department of Veterans Affairs (VA) is seeking a Contractor to assist with the provision of OUTREACH services for homeless Veterans located at the Greater Los Angeles Healthcare System (GLAHS). Duties include: 1.GENERAL REQUIREMENTS: Contractor services shall be furnished to eligible beneficiaries for whom such care is specifically authorized by the referring VA. Responsibilities shall include, but not be limited to: 1.1Outreach Services shall be comprised of 75% Street Outreach and 25% Community Outreach/Events: 1.2Street Outreach- Contractor staff shall be responsible for physically going out into the encampments, under bridges, in parks, etc., and engaging with the Veterans trying to get them off the street, into bridge housing, and into HUD/VASH connected with a VA case manager who will facilitate getting Veteran the services and treatment he/she may need. 1.3Community Outreach- Contractor shall meet Veterans in Veteran Service Organizations (VSO), at community events, churches, libraries, etc., and engage with the Veteran trying to get them into some type of housing(bridge housing and into HUD/VASH) 1.4The focus shall be on reaching the most chronically homeless Veterans as well as those who are not chronic but need housing. 1.5Contractor shall provide a "warm handoff" to GLA HUD/VASH Case Manager. 2.Housing First Training Housing First training will be provided by VA to the contractor upon contract award should the contractor require training in this subject. 3.QUALIFICATIONS- FACILITY/ACCREDITATION /LICENSING/STAFF: 1.The VA is seeking a contract with a Contractor with demonstrated experience providing outreach services to find homeless eligible Veterans. 2.The Contractor may be a direct provider of outreach services or may subcontract with other agencies meeting the same criteria and capable of performing the work outlined herein. 3.The Contractor must be able to provide full staffing as outlined in tasks and deliverables within 60 days of the contract award either through existing staff or new hires. 4.Contractor must have a DUNS number and be registered in SAM. GOV to be eligible for award. This solicitation is for a one year purchase order. Multiple Awards may be awarded in response to this solicitation. This solicitation shall be unrestricted. The solicitation will be available for issue on or about August 28, 2015 with offers due on or about September 14, 2015.
