- 1.GENERAL: This is for a non-personal contract (no services) to deliver fresh bread products to the Department of Nutritional Medicine (DNM) at the San Antonio Military Medical Center (SAMMC). The vendor/contractor must be able to meet and maintain the following conditions: a.Five deliveries per week before 7:00AM b.Three day (72 hours) ordering lead time to include specialty bread items c.Provide same day emergency deliveries not to exceed 3 in one month d.Notify us (the customer) 48 hours in advance if an item is out of stock e.Be able to have access into SAMMC at Fort Sam Houston JBSA, TX f.Include but not limited to the bread products on Exhibit 1 2.Background: The SAMMC DNM provides meals to all patients, staff, and visitors, and it is essential that we have the proper dietary nourishments, to include bread products, in stock. 3.Period of Performance: The period of performance shall be for a base year of 12 months plus 4 option years. The dates for the base year will be known when the contract is awarded. 4.Delivery Address: San Antonio Military Medical Center Department of Nutritional Medicine 3551 Roger Brooke Drive, Building 3600 Fort Sam Houston, TX 78234-6200 5.Security: The vendor is responsible for obtaining access into SAMMC. 6.Delivery Instructions: Deliveries shall be made in the back dock of the SAMMC building #3551 five days per week no later than 7:00AM (Monday thru Friday). Government receiving point of contact is Kevin Lindsey at 210-916-2294. 7.Returns: Credit memorandums shall be issued for returned and/or undelivered items (shortages).
