AMENDMENT 2: Issued on July 28, 2015 to add Attachment 12 to Section J language (Attachment-Past Performance Information form was already loaded to FAACO.) and correct attachment numbers in provision L.3. AMENDMENT 1: Issued on July 28, 2015 to add Addendum 1 as an attachment. Due date for receipt of proposals is not changed. The Federal Aviation Administration (FAA) has a requirement for Credit Union Building Relocation at the Mike Monroney Aeronautical Center (MMAC), Oklahoma City, OK. All work shall be accomplished in accordance to the project specifications and drawings. This project is to relocate the FAA Credit Union into the existing Building #30 on the MMAC campus. The building consists of existing Building #30 (4,770 sq ft) with a drive-thru and remote ATM on Lindy Ritz Lane (formerly SW 64th Street). The building will be modified with additions of an entry vestibule (108 sq ft) and IT, fire riser and restrooms (687 sq ft). Occupancy for this building is 48. The building was designed to incorporate the existing brick/concrete masonry unit (CMU) walls with metal studs, insulation and drywall, existing steel roof structure and Siplast roofing. Ceilings will be existing structure with wood grille system and lay-in ceilings in the remaining building. Floors will have carpet and tile on sealed concrete. Heating and cooling is provided by a new forced air system and lighting is by efficient hanging fluorescent light fixtures that have motion sensors for energy efficiency. It is anticipated that a firm-fixed-price type construction contract will be awarded as a result of this solicitation. Your attention is directed to Sections L and M of SIR/RFO DTFAAC-15-R-05819 for instructions for preparation and submission of proposals. The SIR includes an evaluation of past performance. Offerors are required to provide Past Performance Surveys to customers/contractors who are familiar with the work ethic, standards, performance, and deliverables of the prime/general contractor demonstrated through previous or ongoing contracts of a similar nature [Provision L.3(f)(1).] Offerors will complete Page 1 and Page 2, Section A of the Past Performance Surveys and then provide the surveys to their customers, who will in turn complete and return the surveys directly to the Contracting Officer by the due date specified on the surveys. Three (3) to five (5) Past Performance Surveys must be submitted directly to the Contracting Officer. Surveys received after the established due date may not be evaluated. This requirement is 100% set-aside for competition among Small Businesses. The NAICS code applicable to this requirement is 236220 Commercial and Institutional Building Construction. The business size standard for this NAICS is $36.5 million. A pre-proposal conference/site visit will be held on July 30, 2015 at 9:00 a.m. CST in the Headquarters Building, conference room 148B at the MMAC, Oklahoma City, OK. Attendees should enter the Aeronautical Center's main west entrance gate on South Regina Avenue and proceed to the Security Command Center just inside this entrance. Guards at the Security Command Center will direct attendees to the Headquarters Building. Service trucks will need to enter through the Commercial Entrance on SW 74th Street. ALL QUESTIONS REGARDING THE PROJECT/SPECIFICATIONS/PLANS MUST BE RECEIVED IN WRITING (CONNIE.M.HOUPT@FAA.GOV) NO LATER THAN 4:00 P.M. CST ON AUGUST 14, 2015, TO ALLOW FOR RESPONSES TO BE PROVIDED PRIOR TO DUE DATE FOR RECEIPT OF PROPOSALS. Contractors must be registered with the System for Award Management, www.sam.gov on the date established for receipt of offers (August 26, 2015). As the FAA does not utilize sealed bidding contracting techniques, a public bid opening will not be held.
