- MTC T&C BLANK W-9 FORM MTC VENDOR SELF-CERTIFICATION FORM First Tier Subcontractor Information Form RFQ This is a subcontracting opportunity with Management & Training corporation (MTC), operator of the Flint Hills Job Corps Center. MTC is requesting bids for psychiatric services. Services to be provided are outlined in the attached statement of work. All bids must be returned by mail, electronically or by fax and received no later than 4:00 PM on August 27, 2015. The attached Request For Quote contains the requirements of the project. The First Tier Subcontractor Information form, MTC Vendor Self-Certification Form, MTC Terms and Conditions, and W-9 are provided as well. All forms must be returned with our Bid Sheet in order to be deemed responsive. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTc will be the sole judge of which bid is the best value and its determination and finding on such decision will be final.
