Description
SOURCES SOUGHT The VA Providence Medical Center in Rhode Island has a requirement to provide professional testing, laboratory and inspection services at the site of the new intensive care unit (ICU), project number 650-324. Work includes, but is not limited to, all labor, materials, transportation, apparatus, tools, equipment, and permits necessary to meet the requirement of the project scope of work and project specifications 01 45 29. Master Specification 01 45 29, Testing Laboratory Services, can be located on the Office of Construction & Facilities Management website at http://www.cfm.va.gov/til/spec.asp#02. The purpose of this notice is to determine if there are qualified SDVOSBs, VOSBs, Small Businesses, and Large Businesses that have the experience within a hospital environment for this type of work and if such firms are interested in this procurement. The NAICS code is 541380 and the small business size standard is $15.0 Million. Interested firms should indicate their interest to the Contracting Officer not later than 10am on 07/20/2015 via email to Karla.Rotondo@va.gov. The following information is required in response to this notice: (1) A positive statement of your intention to submit a quote in response to a solicitation for this project; (2) Although other similar services may be required, interested firms shall provide proof that the following testing, inspection and laboratory services can be met with in-house personnel; Earthwork Laboratory Compaction Test Soils Field Density Landscaping Topsoil Test Aggregate Base Laboratory Compaction Test Soils Field Density Gradation Water Soundness Asphalt Concrete Field Density Gradation Wear Soundness Concrete Test Cylinders Compressive Strength Slump Tests Concrete Air Unit Weight Flatness & Levelness Readings Concrete Inspection Reinforcing Steel Tensile Strength Bend Test Mechanical Splice Welded Splice Forms/Rebar Reinf Inspection ? Masonry Test Cubes Compressive Test Cubes Sampling & Testing Mortar C780 Sampling & Testing Mortar C1019 Masonry Unit C140 Prism Tests Masonry Inspector Structural Steel Ultrasonic Testing of Welds Mag Part Testing of Welds Radiographic Testing of Welds Structural Steel Inspection Sprayed-on-Fireproofing Thickness & Density Tests Inspection Technical Personnel (Man-days) (3) Provide proof the two (2) hour on-site emergency response time can be met; (4) Provide four (4) prior completed projects of similar work using the attached past performance form; (5) Firms shall provide proof of valid Joint Venture or Teaming Arrangements, if applicable; (6) SDVOSB or VOSB firms shall be verified by the Center for Veteran Enterprises (CVE), if applicable; and (7) Firms shall hold active SAM registration All information must be submitted in sufficient detail for a decision to be made on the availability of interested firms. Failure to submit all information requested in addition to the below form will result in a contractor not being considered as an interested concern. If adequate interest is not received by 10am on 07/20/2015, this action will not be set-aside for SDVOSBs, VOSBs, or Small Business firms. Interested firms must reply via e-mail to Karla.Rotondo@va.gov. Firms must be registered in the System for Award Management (SAM). Visit www.sam.gov to register in this database. SDVOSB or VOSB firms must be verified by the Center for Veteran Enterprises (CVE).
