Equipment SPECS/Layout of Playground sets SF 1449 Statement of Work DOL Wage Rates Applicable Far Clauses This combined synopsis/solicitation for supply/services is prepared in accordance with the format in subpart 12.6 of the Federal Acquisition Regulation (FAR) as supplemented with additional information included in this notice. This synopsis/solicitation is being advertised as Small Business Set-Aside only. This announcement constitutes the only solicitation; no separate written solicitation will be issued. The solicitation number is HSCG28-15-Q-PMU184. Applicable North American Industry Classification Standard (NAICS) codes are: 238190 Other Foundation, Structure, and Building Exterior Contractor Size standard in Millions ($14.0) This requirement is for a fixed price contract. The contract will be awarded using simplified acquisition procedures in accordance with FAR part 13. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate the proposals: 1. Technical Capability of the services offered to meet/exceed the Governments Requirements per the Attached Statement of Work. 2. Past Performance 3. Price of items offered to the Government Technical and past performance, when combined, is ___60%_______ When submitting your proposals, request a price breakdown of the following: a. Cost of materials b. Cost of labor and supervision to install pour in place playground material c. Cost of Equipment necessary to complete the project Anticipated award date: 10 business days after close of solicitation, OOA 21 August 2015 Quotes are to be received no later than close of business (3 p.m.) on 07 August 2015. Wage determinations: 2005-2461, Rev 17 dated 12/22/2014 SCOPE OF WORK: Contractor is to provide all labor; supervision, materials and equipment to install approximately 4500 square feet of playground pour in place material at Coast Guard Housing "Central Park" located at Coast Guard Airstation Borinquen, Aguadilla PR, per attached SOW. Also attached is the picture of the Concrete pad the playground equipment is/will be attached to and the SPECS/Design layout of the playground structure. Geographical range: Price Range: NTE $67K Performance Period: Supply and install of pour in place playground material will commence after the installation of playground equipment. Commence date to be determined. The Contractor shall provide all labor, supervision and any other supplies, materials and equipment necessary to perform the services required. Site visit: It is highly recommended that prospective bidders attend the scheduled site visit set for Monday 27th of July 2015 beginning at 0930 (9:30 a.m.) to field verify the requirements listed on the Statement of Work.
