This is a sources sought announcement ONLY and is NOT a solicitation for proposals or quotes. The information gathered from this request for information (RFI) is for planning purposes only. The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this Sources Sought Notice. The Orlando VA Health System is seeking business concerns capable of providing emergency ground ambulance services (consisting of Advanced Life Support (ALS), Basic Life Support (BLS) and Specialty Care Transport levels of service) to Veteran beneficiaries of Orlando VA Medical Center Facility addresses are listed below: Orlando VA Medical Center (Lake Baldwin Campus) 5201 Raymond Street Orlando, Fl 32803 Orlando VA Medical Center (Lake Nona Campus) 13800 Veterans Way Orlando, Fl 32827 William V. Chappell Jr. Outpatient Clinic (Daytona OPC) 551 National Healthcare Drive Daytona Beach, Fl 32114-1495 Viera Outpatient Clinic 2900 Veterans Way Viera, Fl 32940 Clermont 805 Oakley Seaver Drive Clermont, Fl 34711 Crossroads Annex 925 South Semoran Blvd Suite 114 Winter Park, Fl 32792 Kissimmee Community Based Outpatient Clinic 2285 North Central Avenue Kissimmee, Fl 34741 Leesburg Community based Outpatient Clinic 711 W. Main Street Leesburg Fl 34748 Orange City Community Based Outpatient Clinic 2583 South Volusia Ave (17-92) Suite 300 Orange City, Fl 32763 Ambulance services required by Veteran beneficiaries include the provision of non-emergent ambulance transport services and wheelchair transport services in the Central Florida location in the following counties: Orange, Brevard, Lake, Volusia, Seminole and Osceola 365-days per year, 24-hours per day, 7-days per week. BLS transport shall include on-board EMT (Emergency Medical Technician) and basic airway management services as appropriate. ALS transport shall include an on-board paramedic or RN (Registered Nurse) as appropriate, (ECG) Electrocardiogram, pressure monitoring, suction, ventilator, use of resuscitation equipment including defibrillator, provision of ACLS (Advanced Cardiac Life Support) drugs and procedures, advanced airway management and medication monitoring and administration, to include multiple drips such as IV heparin and nitroglycerin. Contractor shall provide all necessary equipment for transport of ALS and BLS patients. Ambulances shall meet all current applicable Federal, State and local specifications and regulations including, but not limited to, licensing, registration, and safety standards. Appropriate licensing by the State of Florida is required. All Contractor personnel performing contract services shall meet all qualifications and licensures required by Federal, State, County and Local Government entities from the place in which they operate. Nurses assigned by the Contractor to perform services shall be Licensed Registered Nurses in a State, Territory, or Commonwealth of the United States or the State of Florida. EMTs and Paramedics shall have completed training in accordance with the standards published by the Department of Health and Human Services with a minimum curriculum of 150 hours or equivalent including an in-hospital training period. Drivers shall have a valid operator's license in accordance with Federal, State, County and Local Government requirements for the subject counties for the services they perform; shall be capable of administering oxygen and shall have successfully completed Standard and Advanced First Aid Courses. Firms with interest and capability to include, Federal Supply Holders, Small Business, Veteran-Owned Small Business, Service-Disabled Veteran-Owned Small Business, HUBZone Small Business, Small Disadvantaged Business, and Women-Owned Small Business Concerns may respond by submitting a qualifications letter stating the following information: 1.Name 2.Capability Statement (to include the Counties/Locations of interest) 3.DUNS # 4.Type of business (i.e., SDVOSB, VOSB, WOSB, etc ¦) Responses to this sources sought notice shall be sent via email, no later than 11:00 AM July 31, 2015.
