- **NOTE: The small business set aside for Lot 3 is dissolved. Lot 3 is now unrestricted. The proposal due date for Lot 3 is extended to July 31, 2015 at 4:00PM PST. Please note Amendment A00005 applies only to Lot 3. Proposals received for any Lot other than Lot 3 will not be considered for award.** Network Contracting Office 21 (NCO 21) plans to issue a Request for Proposal (RFP) for ground ambulance services for the San Francisco Veterans Affairs Medical Center (SFVAMC), Veterans Affairs Palo Alto Healthcare System (VAPAHCS), and Veterans Affairs Northern California Healthcare System (VANCHCS). Travel will include ALS, BLS, or CCT services for one-way and/or roundtrip transportation to or from the medical centers listed above to a hospital or medical facility within the State of California. The solicitation number for this RFP is VA261-15-R-0057. The solicitation will be issued on or about April 17, 2015, with an anticipated proposal due date of May 18, 2015. The Period of Performance of the Base Year begins on or about October 1, 2015 through September 30, 2016 with four (4) one year option periods. The North American Industry Classification Systems (NAICS) is 621910, size standard $15 million. There will be multiple single Indefinite Delivery Indefinite Quantity (IDIQ) awards based on Geographic Lots. IDIQs are funded by task orders and each task order is firm fixed price. The anticipated Lots are as follows: Lot 1 - Humboldt County Lot 2 - Mendocino County Lot 3 - Sonoma, Marin, Napa, San Francisco Counties Lot 4 - Solano, Sacramento, Contra Costa Counties Lot 5 - San Joaquin, Stanislaus Counties Lot 6 -Tuolumne County Lot 7 - Santa Clara, Alameda, San Benito, San Mateo, Santa Cruz, Monterey Counties Lot 8 - Shasta County Lot 4 is 100% set aside for small business. Contractors must be registered in the System for Award Management (SAM) at www.sam.gov. All questions shall be submitted in writing to Nicole.do@va.gov not later than May 13, 2015 at 4:00 PM HST. Note: SDVOSB firms must be registered at the time of quote submission and CVE verified in VetBiz Registry http://www.vetbiz.gov/ Vendors not registered in the VetBiz Registry at the time of quote submission will not be considered for award.
