Description
This is a REQUEST FOR INFORMATION (RFI) ONLY to provide non-emergent ambulette services to the Department of Veteran Affairs, VISN 11, Toledo Community Based Outpatient Clinic (CBOC). DO NOT SUBMIT A PROPOSAL in response to this request for information. This is only a market research effort at this time. The contractor shall be able to respond to the Toledo Community Based Outpatient Clinic, 1200 Detroit Avenue, Toledo, Ohio within thirty (30) minutes. The contractor shall have the necessary equipment and personnel to respond to several requests daily. The contractor shall comply with all Federal, State, and/or City codes and regulations regarding operations of an ambulette service. Contractor must be licensed by the State of Ohio to operate an ambulette in accordance with Ohio Revised Code 4766. Contractor shall transport patients to or from any pickup points, to or from any floor, ward, nursing home, patient(s) home, medical center, or facility, Monday through Friday, excluding Federal holidays during the hours of 6 AM to 6 PM. Occasionally, transportation will be required prior to 6AM or after 6 PM Monday through Friday, on weekends and Federal Holidays. The Toledo CBOC proposes to provide the contractor with a minimum of twenty-four (24) hours advance notice of these transportation requirements. There may be some instances where 24-hour notice is not feasible and contractor would be expected make every reasonable effort to accommodate these requests. The Government requests that interested and capable contractors provide a capability statement. Interested parties may submit a capability statement, no more than one (1) page, for your company (name, mailing and email address), Dunn and Bradstreet number, any subcontractors and also denote the business size of your company under the North American Industry Classification System (NAICS) code 621910 Ambulance Services. Also, indicate if your company or subcontractors are Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a) or Woman Owned Small Business (WOSB). Responses to this RFI are requested to be submitted not later than 12:00 NOON EST on July 9th, 2015, via email to roy.samsel@va.gov. Please title the email "Toledo Ambullete Capabilities"
