EST, July 16, 2015 Description This is a Sources Sought Announcement. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this RFI is to obtain knowledge and information for acquisition planning purposes. The Department of Veterans Affairs Network 11 Contracting Office is conducting market research to identify potential sources that meet the requirements generally described in this notice. The intent of this announcement is to: -Notify potential sources about the requirement -Identify interested vendors The Department of Veterans Affairs, Network 11 Contracting Office, on behalf of the Richard L. Roudebush VA Medical Center in Indianapolis, IN is seeking to acquire a comprehensive preventative maintenance and repair plan for the Biosense Webster Carto 3 System located at the Medical Center at 1481 West 10th Street, Indianapolis, IN 46202-2803. The Government wishes to have a plan for a base year plus one option year for a total of two years of service. Information Requested Vendors interested in this RFI may provide a response that includes information on your firm's capability to provide a the required services as written in the attached DRAFT Statement of Work to the Richard L. Roudebush VA Medical Center in Indianapolis, IN. Please also provide your firm's DUNS number, organization name, address, business size and type as validated via the Central Contractor Registry (CCR) (e.g., 8(a), WOSB, VOSB, SDVOB, or HUBZone small business) pursuant to the applicable NAICS code. Respondent's points of contact, including names, titles, addresses, telephone and fax numbers, and e-mail addresses. Instructions for Submittal Written responses shall be provided electronically, by 12:00 p.m. EST on July 16, 2015. The subject line of the e-mail message shall read: "Biosense Webster Carto 3 Sources Sought" and shall be addressed to the attention of: jared.tritle@va.gov The VA will not accept telephone, facsimile, courier, mail, or hand delivery methods of response. The page limitation is 5 pages. Corporate capability information used as marketing material and commercial price list information are in addition to the 5 page limit and may be submitted as separate files. Disclaimer and Important Notes This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. Any solicitation resulting from the analysis of information obtained will be announced to the public in Federal Business Opportunities in accordance with the FAR Part 5. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s). ? DRAFT STATEMENT OF WORK SCOPE OF WORK: The Contractor shall provide a comprehensive service plan, to include preventative maintenance and repair services, for the Biosense Webster manufactured Carto 3 System located at the Richard L. Roudebush VA Medical Center at 1481 West 10th Street, Indianapolis, IN 46202-2803. CONFORMANCE STANDARDS: All services provided under this contract must be performed in conformance with the Original Equipment Manufacturer and Occupational Safety and Health Administration (OSHA) standards and specifications. HOURS OF WORK: Hours of Work for preventative maintenance and repair services are defined as Monday through Friday from 7 a.m. to 5 p.m., excluding federal holidays, or as otherwise arranged with the Contracting Officer Representative. The ten holidays observed by the Federal Government are New Year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday. Work performed outside of normal working hours of coverage at the request of the Contracting Officer or Contracting Officer's Representative shall be billed separately. The Contractor shall not perform work outside the scope of this contract without first receiving written permission from a Government Contracting Officer. REPAIR SERVICE: Repairs shall be covered (during normal working hours) under this service contract for the Biosense Webster Carto 3 system in order to maintain the system at Manufacturer's specifications. The Contractor shall perform routine quality assurance evaluations in order to assure optimum performance of the software. If technician takes a first look and calls Biosense Webster to gain permission, parts may be sent out to be repaired. PREVENTATIVE MAINTENANCE SERVICE: The Contractor shall provide software updates/upgrades and hardware upgrades to the Carto 3 which are intended to correct a safety risk. Software updates offering enhancements to previously purchased software features shall be covered and provided by the contractor (if hardware modifications or additions are not needed). Contractor will provide all necessary software updates in order to keep software current and running smoothly. PERFORMANCE MONITORING, DOCUMENTATION, & REPORTS: The Contractor shall submit a legible field service report immediately following the service visit, which shall include detailed descriptions of the maintenance performed or emergency repair services performed. The field service report shall include replaced parts and estimated prices required for the service call. Any additional charges not included within this service contract shall have the prior approval of the Contracting Officer Representative before those costs are incurred. Any costs incurred without obtaining prior approval may be determined by the VA to be unauthorized work and the contractor may bear the burden of such costs. COMPETENCY OF PERSONNEL SERVICING EQUIPMENT: All repair and maintenance services shall be performed by a Field Representative who is fully trained on the Toshiba equipment under this service contract. The contractor shall provide written assurance of the competency of their personnel by detailing a list of Field Service Representatives along with applicable training and years of service experience on the specific devices covered in this service contract. This information shall be included in the quoted proposal for providing service. SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken.
