- Attachment 1 - Pricing Matrix Statement of Work RFQ-HSBP1015Q00107 - SF1449 (1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (2) The solicitation number is HSBP1015Q0107 and is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-78. The associated North American Industrial Classification System (NAICS) code for this procurement is 541940 with a small business size standard of $7.5 million. (3) This procurement shall be set-aside for small business. All qualified, responsible sources may submit an offer that shall be considered by the agency. Offerors are advised that they are responsible for obtaining amendments. The resulting contract shall be awarded to the offeror who provides the lowest price technically acceptable. FOB destination shall be Pharr, Texas. (4) The Department of Homeland Security (DHS), Customs and Border Protection, intends to award a Blanket Purchase Agreement for the procurement of Veterinarian Services and canine supplies. It is required that all contractors submitting a quote review the attached STATEMENT OF WORK. (5) Period of performance shall be 08/01/2015 - 07/31/2020. (Dates subject to change). (6) Attachments: a. RFQ: HSBP1015Q00107, with terms and conditions b. Statement of Work c. Attachment 1 - Pricing Matrix (7) All contractors must complete and submit the following 2 documents via email to susan.gonzalez@dhs.gov: 1. Page 1 of the RFQ, completed block 30A - 30C 2. Attachment 1 - Pricing Matrix, completed with bid pricing. (8) Questions shall be submitted in writing via email to: susan.gonzalez@dhs.gov.
