complete synopsis/solicitation including SF-1449, FAR Clauses and Solicitation Provisions Experience Questionnaire form DOL Wage Determination Schedule of Items Quote form to submit pricing with SF 1449 Lost Horse Gravel Schedule of Items 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in SubPart 12.6 of the Federal Acquisition regulations (FAR) as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The awarded contract will be firm fixed price. 2. This solicitation is issued as a Request for Quotation (RFQ). 3. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66B. 4. The solicitation is being issued as a Total small Business Set-Aside. The associated NAICS code is 484220 Specialized Freight (eccrpt Used Goods) Trucking, Local. The Small business size standard is $27.5 million. 5. The combined synopsis/solicitation is for the acquisition of the following commercial items: gravel hauling using three End Dump trucks. Contractor is required to be registered in System for Awards Management at www.sam.gov (formerly known as Central Contractor Registration (CCR) database and Online Representations and Certifications Application (ORCA) before award. CONTRACT TIME The contract period will be approximately July 13, 2015 - July 31, 2015. BASIS OF AWARD Offer of award will be made to the quoter whose quote, as determined by the Contracting Officer, provides the best value to the Government, considering but not limited to such factors as price, Contractor experience with this type of work, availability to start work on July 13 and commit to the expected three week duration, and past performance in accordance with FAR 13.106-2. This is a service contract, Service Contract Act applies,. 2. Under Clause 52.236-7, Permits and Responsibilities, Contractors shall be responsible for complying with any Federal, State, and municipal laws, codes and regulations applicable to the performance of work. 3. Hauling Schedule and hours of hauling operations: Monday through Thursday only, from 0730 - 1730. See Attachment: Schedule of Items CONTRACT TIME The contract period will be approximately July 13, 2015 - July 31, 2015 SITE VISIT None PREWORK CONFERENCE None SCOPE OF CONTRACT The purpose of this contract is to provide the following: gravel hauling and spreading of government furnished gravel in cooperation with the Forest Service road crew, Bitterroot National Forest, Darby, Montana area. PROJECT DESCRIPTION AND LOCATION a) Description of Work This work is to be accomplished in cooperation with the Forest Service road crew. Work includes, but is not limited to, hauling and spreading government furnished gravel for the Forest Service grader operator Contractor shall provide three (3) end dump trucks (minimum ten (10) yard capacity) and drivers. Hauling operations shall be accomplished Monday thru Thursday only, from 0730 - 1730. No gravel shall be dumped on roadway without Forest Service grader operator on site to spread/shape. b) Project Locations 1. Point of load: Lost Horse Pit - From Hamilton City Center (First and Main intersections) drive 9.5 road miles south on U.S. Highway 93. Turn right on Lost Horse Road. Drive 4.2 miles west on Lost Horse Road to Lost Horse gravel pit. Pit entrance and gravel stockpile is on left (south side of Lost Horse Road). Pit is located on the Darby Ranger District in Section 18, T 4 N, R 21 W 2. Points of discharge: gravel shall be spread over various sections of Lost Horse Road ranging from three (3) miles up to fifteen (15) miles from pit location. START WORK It is a requirement under this contract that work begin on site July 13, 2015 (See Part I, Section F, Clause 52.212.3) GOVERNMENT-FURNISHED PROPERTY The Government will deliver to the Contractor materials, supplies, property or services (hereinafter referred to as "Government-furnished property") as listed below. The Contractor shall be liable for all loss or damage of such delivered Government-furnished property until completion and final acceptance of work required under this contract. If the Government fails to make timely delivery of such government-furnished property suitable for its intended use and upon written request from the Contractor, the Contracting Officer will make an equitable adjustment of contract delivery or performance dates or contract price or both. The Government will furnish the following items: Crushed Aggregate Source (gravel) - Lost Horse Pit: M.P. 4.2 of Lost Horse Road, NFSR 429 F-1- COMMENCEMENT, PROSECUTION AND COMPLETION OF WORK (FAR 52.212-3) APR 1984) The contractor shall be required to: a) Commence work under this contract within 4 calendar days after the Contractor receives the Notice to Proceed: b) Prosecute the work diligently in accordance with Section 156 and complete the entire work ready for use not later than July 31, 2015. c) The time stated for completion shall include final cleanup of the premises. Requirement: Hauling operations shall be accomplished Monday thru Thursday only, from 0730 - 1730.
