Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The Government intends to issue a purchase order in accordance with FAR Part 13 for using Simplified Acquisition Procedures. The solicitation number is VA247-15-Q-0747. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-70 effective September 30, 2013. The associated North American Industrial Classification System (NAICS) code for this procurement is 333413 and the Small Business Size standard is 500 employees. This combined/synopsis solicitation is set aside for small business. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items. The Department of Veterans Affairs, Birmingham Community-Based Outpatient Clinic (CBOC) provides state-of-the-art imaging testing in support of patient care for women Veterans. In order to support the demands derived from patient care and imaging testing, we must have the proper storage to support the patient imaging workload. We are looking for two (2) Ultrasound Probe Cabinets to store up to six new endo-vaginal probes along with the replacement filters. This procurement is BRAND NAME OR EQUAL pursuant to FAR 52.211-6 and VAAR 852.211-73. The requested product contains Model #CAB-VR. Bidders quoting an Equal product are required to meet the below specifications and submit any product information pertaining to the requested item. The probe storage system must meet the following specifications- "Vertical Storage Cabinet "Joint Commission approved "True HEPA Filter and Low Noise Fan to Provide Continuous Airflow "Filter Change Indicator Light "Specialty Shelves to Protect from Cross-Contamination "Large Viewing Window "Locking Door for Added Security "Holds up to six (6) vaginal, rectal and general purpose probes The RFQ period closes on Friday, June 12, 2015 at 5:00PM EST.
