- Attachment 5 - Wage Determination Attachment 4 - Management Questionnaire Attachment 3 - Sample Menu Attachment 2 - Award Fee Plan Attachment 1 - Performance Work Statement Solicitation Package DJF-15-1200-R-0000051 *AMENDMENT* The following information: "All interested parties should submit a written request for RFP DJF15-1200-R-0000051 by email, referencing the solicitation number in the SUBJECT line of the email with a central Point of Contact to include a phone number, to the Contract Officer Jennifer L. Robinson at Jennifer.Robinson@ic.fbi.gov. Request for solicitation may also be sent by United States Mail, postmarked no later than 15 days from the date of this notice to Federal Bureau of Investigation, CJIS Division, Attn: Jennifer Robinson, 1000 Custer Hollow Rd, Bldg CMT-208, Clarksburg, WV 26306. Any requests emailed or postmarked after the 15 days will not be subject to the availability of the solicitation package. Telephone requests will not be honored." SHALL BE AMENDED TO READ: "All interested parties shall obtain solicitation documents through FedBizOpps.Gov to include any amendments." "The FBI is issuing the RFP for Cafeteria Services which consists of Front End Documents and five (5) attachments as referenced in Section 4 of the RFP. Please take into consideration all additional Security Requirements as outlined within the solicitation as they may not be outlined in "Instructions to Offerors". This proposal shall be structured in accordance with the instructions provided with this solicitation package. PRE-PROPOSAL SITE VISIT CONFERENCE: All offerors who plan to submit an offer are strongly urged to attend the pre-proposal site visit conference that is scheduled for Wednesday, June 17th, 2015 at 1:00 PM. If you plan to attend this site visit, please provide the following information for access onto the CJIS Campus for each individual attending (Limit no more than four (4) per Contractor). - Full Legal Name - Social Security Number - Place of Birth - Date of Birth Information must be provided no later than 1:00 PM Friday, June 12th, 2015 to Stephen Mayers via email at Stephen.Mayers@ic.fbi.gov. This information will be vetted through security for access to the CJIS facility. All attendees must have a valid driver's license or other state issued identification to gain access onsite. Should access be denied, you will be notified by email. In addition, a list of all attendees should be emailed to the Contracting Officer by the same due date and time on June 12th, 2015 at Jennifer.Robinson@ic.fbi.gov. SPECIAL NOTE: The FBI CJIS Division Site is located wihin the city limits of Clarksburg, West Virginia. For any work performed on the site the Contractor shall be responsible for obtaining a City of Clarksburg, WV, business license in order to meet Business and Occupation Tax obligations. The Contractor shall take this into consideration for the purpose of including these fees and taxes in their proposal. End of Amendment
