Kingsbridge Road Rm. 9b16 Bronx NY 10468 Synopsis: Please note that this is not a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Saint Albans VA Medical Center, is conducting a market survey for qualified firms capable of providing the following services for Saint Albans VAMC. Laundry Computer Software Update Vendor shall be engaged in maintenance training as well as servicing laundry computer software, meeting the manufacturer specifications to maintain it in first class operating condition. SERVICE TO BE PROVIDED: 1. Monthly Service to Include: Three (3) days on site consisting of the last day and first analysis reports, database back network maintenance and system troubleshooting as needed. 2. TERMS: Contractor shall provide services toVA pursuant to this agreement for a term commencing four (4) option years as stated in pages herein. 3. PLACE OF WORK: Contractor shall render services primarily at contractor's offices, but will, upon request provide the service at VA Office of such other places as reasonable requested by VA as appropriate for the performance of particular services. 4. TIME: Contractor's daily schedule and hours worked under this agreement on a given day shall generally be subject to contractor and VA anticipates that contractor shall work on average 24 hours in the performance of service pursuant to this agreement. VA relies upon contractor to devote sufficient time as is reasonably necessary to fulfill the spirit and purpose of all service performed pursuant to this agreement. Payment shall be made in monthly installments. Contract shall bear all of contractor's expenses incurred in the performance of this agreement. 5. INDEPENDENT CONTRACTOR: Contractor is and throughout this agreement shall be an independent contractor and not an employee, partner or agent of VA. 6 TOOLS AND SUPPLIES: Contract shall be solely responsible for procuring, paying for and maintain any computer equipment, software, paper, tools, or supplies necessary or appropriate for the performance of contract services hereunder. 7. FINAL AGREEMENT: This agreement constitutes the final understanding and agreement between the parties with respect to the Subject matter hereof and supersedes all prior negotiation, understanding and agreement between the parties whether written or oral. 14. NOTICES: Any notice required to be give of otherwise given pursuant to this agreement shall be in writing and shall be hand delivered, mailed by certified mail, return receipt requested or sent by recognized overnight courier service as follows: If to contractor: Interactive Control Systems 5081 State Hwy 7 Marlin, Texas 76661 If to company: St. Albans VA Textile Care Facility 179th Street an Linden Boulevard St. Albans, New York 11425. 15. SEVERABILITY: If any term of this agreement is held by a court of competent jurisdiction to be invalid for unenforceable, then this agreement including all of the remaining terms will remain in full force and effect as if such invalid or unenforceable term had never been included. The North American Industry Classification Systems (NAICS) is 541519. Responses to this notice must be submitting in writing via email and must be received no later than June 12, 2015. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources for this procurement relative to NAICS 541519. Qualified offerors shall be experienced in addition to providers (sellers) capable of supporting this facility. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, and examples of similar facilities which you have provided services to DUNS number, address, and point of contact. If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Responses to this notice must be submitted in writing (email or fax) and received no later than June 12, 2015 at 3:00PMEST. A solicitation is not currently available.
