- Tax Exempt Certificate Purchasing Terms and Conditions New Vendor Form The Westover Job Corps Center invites vendors to submit bids on the following service, for the remaining Contract period 5/1/15 through 2/29/16. Total Student Population is 437 students. DESCRIPTION OF SERVICE: Subcontractor will provide a full and thorough eye examination for students. Exam must include inspection of sclera, lids, conjunctiva, cornea, and eye position. Subcontractor will provide quality laboratory services to fill prescriptions for glasses, replace broken lens, and adjust and repair frames as required. All eyeglasses must meet American National Standards Institute (ANST) standards. Subcontractor will permit the students to select any style that does not exceed the schedule price. Students who select frames in excess of the schedule price will be responsible for payment of additional charge. The subcontractor will bill the Westover Job Corps center for only those costs approved and outlined on the fee schedule. Contact lenses will be provided only when improvement on vision would be substantially greater than that achieved from glasses. The subcontractor will obtain verbal approval from the Wellness Manager prior to prescribing contact lenses. The Westover Job Corps Center will refer all students with refractive errors of 20/40 or worse on the Snellen Chart and/or J/6 or worse on the Jaeger or similar chart. SCHEDULE OF PERFORMANCE: Weekly appointments are scheduled. Bids must be received either by FAX or E-MAIL by April 24, 2015 at 5:00 EST. The Westover Job Corps Center reserves the right to accept or reject any and all bids. Small businesses and minorities are encouraged to reply. Vendors must provide Certificate of Insurance, W9, and confirmation of registration on SAMS.Gov. Vendors must also become approved vendors by completing/signing the attached documents.
