- This is a Request for Information and Sources Sought Notice for Market Research purposes only. This is not a request for proposal, quotation, or bid. No solicitation is currently available. The United States Property & Fiscal Officer (USPFO) for South Carolina is seeking information regarding capability and availability of potential business sources in support of a potential requirement to provide Medical Non-clinical Case Management in support Individual Medical Readiness (IMR) throughout the state of South Carolina. In the event of a solicitation, we anticipate a negotiated firm fixed price award for a duration of five years: the Base Year and four Option Years, pending the availability of funding and assurance of quality performance of services by the contractor. The period of performance shall be 12 June 2015 - 11 June 2016 for the Base Year. The primary type of work will include non-clinical medical and administrative assistant/care coordination in support of IMR. The primary performance location is in Eastover, SC with secondary locations in South Carolina. However, the place of performance varies when providing support for off-site events held on weekends as the medical staff performs services at National Guard armories throughout the state during Unit Training Assemblies (UTA); otherwise known as Drill. Department of Defense Instruction 6025.19 establishes aggressive quarterly and annual metric goals for the separate IMR elements and for the overall IMR category of Fully Medically Ready (FMR) soldiers. The ultimate goals are to produce real-time reports based on complete, accurate data. The minimum goal for the overall medical readiness is more than 75% of Service Members FMR, with the ideal goal being 100%. The SCARNG Office of the State Surgeon requires a highly responsive process whereby the Government can quickly obtain needed support and services. Any resulting solicitation shall require both case manager and administrative care coordinator support services. It would likely include qualification requirements, such as Registered Nurse (RN) and Licensed Vocational/Practical Nurse (LVN/LPN) or Social Worker. It would include Army Regulation (AR) 40-501 and other applicable guidance. The SCARNG is seeking qualified 8(a) or HUBZone small businesses as defined in the Federal Acquisition Regulation (FAR) Part 19 that qualify under the North American Industry Classification System (NAICS) Code 561330 (Professional Employer Organizations, size standard is $27.5 MIL). Responses should include the company name, address, contact name and title, phone number, e-mail address, business size and classification/certification under the NAICS Codes listed above, Commercial And Government Entity (CAGE) Code, Dun & Bradstreet (D-U-N-S) Number, and a list of your company's three (3) most recent contracts/projects of the same type described in this notice, if applicable. Please limit responses to four (4) pages or less. This notice does not constitute a request for a formal proposal. This notice is provided as information to the market place and is an invitation for an expression of interest and brief demonstration of qualifications to perform and deliver the anticipated requirement. The Government will not pay for the provision of any information, nor will it compensate any respondents for the development of such information.
