That RFP and its attachments are for reference purposes only. Only the RFP and its attachments found on this posting should be used when creating a response. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. This solicitation, CC15HQR0009 is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions amd clauses are those in effect through Federal Acquisition Circular 2005-78, effective December 26, 2014. This is an unrestricted procurement and the associated NAICS code is 524113, Direct Health and Medical Insurance Carriers with a size standard of $38.5 million. The Office of the Comptroller of the Currency (OCC) is issuing an RFP using FAR PART 12 - Acquisition of Commercial Items and FAR Part 15 - Contracting by Negotiation to obtain an experience and qualified contractor to underwrite and administer its short-term and long0term disability insurance program for the OCC and the Office of Financial Research (OFR) employees. The contractor shall administer these programs from an integrated disability management approach to ease the transition from STD to LTD. This integrated approach should enhance the employees' overall experience that aids in the return to work efforts, vocational rehabilitation, claim filing process, and early social security notice. A detailed PWS is included in the RFP. The OCC intends to award a firm-fixed-price standalone contract with a base (to include transition, if applicable) plus four one-year option periods. It is the offeror's responsibility to monitor FBO for the release of any amendments that may follow. The provision at FAR 52.212-1, Instructions to Offerors - Commercial, and the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items, apply to this acquisition. The provision FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: 1. IT security compliance 2. Technical Capability 3. Experience 4. Past performance 5. Price Factor 1 is a pass/fail factor. Receipt of a "fail" rating for the security compliance evaluation factor will result in exclusion from further consideration of award. Factors 2-5 are listed in descending order of importance; however, when all factors non-price factors are combined, they are significantly more importance than price. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition and can be found in full text in the RFP along with all other applicable FAR clauses. Questions are due on March 9, 2015 on or before 10:00 A.M. E.T and the deadline for receipt of proposals is March 26, 2015 on or before 10:00 A.M. E.T. Proposals received after the submission date and time may not be considered for award.
