- Attachment 1 Statement of Work Request For Quotation SP330015Q0028 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 - Streamlines Procedures for Evaluation and Solicitation for Commercial Items in conjunction with the Federal Acquisition Regulations (FAR) Subpart 13. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the RFQ number is SP330015Q0028 (see attached). •2. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-78 effective 26 December 2014 and Defense Federal Acquisition Regulation Supplement (DFARS) DCN 20150115 edition and Defense Logistics Acquisition Directive (DLAD) Current to Revision 5 and PROCLTR 2015-07. The complete text of any of the clauses and provisions may be accessed in full text at http://farsite.hill.af.mil. 3. This Request for Quotation (RFQ) is being issued to establish a firm-fixed price contract for heater rental. Contractor shall provide three (3) 1,000,000 BTU stand-alone indirect fired heaters to heat warehouses building 404 and 405 located DLA Distribution Barstow, CA in accordance with Statement of Work (SOW) provided in Attachment 1. The rental period shall run from February 01, 2015 through March 31, 2015 (2 months) with an option to renew for the following heating season, which will run from November 15, 2015 through March 31, 2016 (4.5 months). 4.This RFQ will be awarded on a 100% small business set-aside. The North American Industry Classification System (NAICS) code for this project is 334512 and the size standard is 500 employees. •5. 5. PRICING SHALL BE FOB DESTINATION. 6.Required Delivery: 15 days after date of award or sooner. •7. 7. Delivery Information: The items shall be delivered as follows: DLA Distribution Barstow DODAAC: SB3208 Buildings 404 & 405 Barstow, CA 92311 8. The method of payment under this order is Wide Area Work Flow (WAWF). See DFARS clauses 252.232-7003 and 252.232-7006 below. 9. 9. SYSTEM FOR AWARD MANAGEMENT - A prospective awardee shall be registered in the System for Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.sam.gov. 10.
