Description
Title: Retrofit and Refurbish X-Ray Mobile Vehicle The Centers for Disease Control and Prevention (CDC) and the National Institute for Occupational Safety and Health (NIOSH), Division of Respiratory Disease Studies, Enhanced Coal Workers' Health Surveillance Program intends to award a sole source firm fixed price purchase order to LifeLine Mobile, Inc., 2050 McGaw Road, Columbus, Ohio 43207 to retrofit and refurbish the NIOSH enhanced Coal Workers' Health Surveillance Program's mobile x-ray vehicle. Retrofitting to move interview area and add second spirometry area; refurbishing to include new ceiling tile, baseboards, cabinets, touch-up paining, new graphics, shelving, etc., as noted on in the statement of work below. Work to be completed by May 22, 2015. STATEMENT OF WORK We are requesting vendor to retrofit and refurbish the NIOSH Enhanced Coal Workers' Health Surveillance Program's Mobile X-ray Vehicle. The mobile in now 9 years old and is in need of a refurbishment. In addition, the retrofit to add a second spirometry area will aid in a faster turnaround time for the participants in the program. Retrofitting will move interview area and add second spirometry area, and refurbishing to include the following items to be repaired, replaced, and/or installed: - Repair and update storage attic door hinges - Add auto transfer switch system with protection when changing from shore power to generator power - Remove old chairs and install air ride captain's chairs in cab - Upgrade headlights and fog lights to enable safer night driving - Add new graphics on the exterior and repair existing artwork - Replace baseboards throughout - Replace cabinets throughout - Remove current waiting area slide out furniture, and install desk and bench storage - Install tall storage cabinet - Remove waiting area and install a 2nd spirometry room with access door - Add valances and replace blinds throughout - Touchup paint throughout - Replace ceiling tiles throughout - Add shelving to attic storage area - Remove all rust and repaint at tanks, under attic, and around slide out - Remove attic exterior panels to eliminate oxidation around rivets - Replace slide out seals - Repair undercarriage hinge - Minor repairs throughout, including repairing slide out water leak and battery charging Period of performance: Work must be completed by May 22, 2015. This contract action is for a product for which the government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b) and 10 U.S.C 2304(c) (1). Interested persons may identify their interest and capability to respond to the requirement or submit proposals. The notice of intent is not a request for competitive quotations; however, all quotations/responses received within (15) days of the issuance of this notice will be considered by the Government. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
