Description
The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 8), James A. Haley Veterans Hospital, Tampa, FL is currently seeking information from any vendor that can provide the following service. Non-Emergency Special Mode Transport Background: Contractor shall furnish 24-hour non-emergency transportation for wheelchair, stretcher, and other ambulatory service to beneficiaries of the James A. Haley Veterans' Hospital, 13000 Bruce B. Downs Blvd., Tampa, Florida 33612 and New Port Richey Out Patient Clinic New Port Richey, Florida 34654. The contractor shall provide all manpower, equipment, materials, supplies, vehicles, fuel, and supervision necessary to perform all tasks. All trips will be either originating from, or going to, the James A. Haley Veterans' Hospital, Tampa, FL and New Port Richey Out Patient Clinic New Port Richey, Florida 34654. Non-emergency ambulance transportation services shall be furnished on an "as needed" basis. Coverage is required 24 hours per day, seven days per week, including holidays. Qualifications: The Contractor shall meet all requirements of Federal, State and City code regarding operation of this type of business. The contractor shall be licensed by the State of Florida Department of Health, Bureau of Medical Services to provide non-Emergency Medical Services. The contractor shall maintain licensure/certification/Certificate of Public Convenience Necessity (COPCN) with the State of Florida, and Hillsborough County, as applicable. The contractor shall maintain documentation demonstrating they meet all requirements of Federal, State, County, or City codes regarding operation of this type of service. The contractor shall comply with the provisions of the Privacy Act of 1974, the Health Insurance Portability and Accountability Act (HIPAA) of 1996. Contractor must carry all necessary insurance coverage required as required by the contract and Federal, State, County or City codes. Personnel and Equipment: The Contractor shall ensure all Emergency Medical Technicians (EMT), Paramedic, Non Emergency Attendant/ Drivers training requirements are met. The ambulances used under the terms of this contract shall be licensed and meet the minimum requirements governing emergency vehicles that is mandated by Florida Department of Health, Bureau of Emergency Medical Services. Every ambulance in service under this contract shall be equipped as required by the State of Florida and carry at least the minimal equipment necessary for the type of service to be provided, as determined by standards adopted by the State of Florida. Each emergency care vehicle shall have patient compartment facilities, oxygen and suction systems and equipment, environmental climatic equipment, communications and additional systems equipment accessories and supplies as required by the applicable codes, rules and regulations established by Federal, State and Local regulating authorities. Ordering Service: James A. Haley Veterans' Hospital beneficiary travel personnel will place scheduled and unscheduled transportation requests via telephone, fax, or email of the request to the Contractor's dispatch office. The request for services shall specify the originating point, final destination, and any special needs required. Only such travel specified is authorized. The contractor shall provide all routing directions to drivers prior to arrival at the James A. Haley Veterans' Hospital or New Port Richey Out Patient Clinic New Port Richey, Florida 34654. Each request shall have an identified VA authorization number associated with the trip request. When transporting patient(s) leaving the hospital, the Contractor may be required, if necessary, to transport a maximum of three (3) items (to include boxes, luggage, and/or durable medical equipment) at no additional cost to the VA. Origin and Destination: Patients may be transported to or from any designated location within the defined service area, encompassing the state of Florida and South Georgia. All transports will be between VA facilities, between a VA facility and a facility that provides care under VA auspices (i.e. Contract Nursing Home, Community Outpatient Clinics) and/or between the home of the beneficiary receiving travel services and the VA facility. VA staff will specify the points of origin and the destination of every trip. Response times will be based upon whether the transportation requested is Standard or Urgent/STAT. Unless otherwise agreed upon, a Standard response time is within 45 minutes. An Urgent/STAT response time is within 30 minutes or sooner. Interested Vendors The applicable NAICS for this service is 485991. To be considered for this requirement, vendors must be registered in the System for Award Management (SAM - https://www.sam.gov) and be identified as a licensed service provider for ambulance service on the Hillsborough County Public Transportation Commission website (http://www.hillsboroughcounty.org/index.aspx?nid=2595). If a vendor is not currently identified as a licensed service provider, they must be able to successfully register prior to submitting any offer associated with this requirement.
