Description
The purpose of this notice is to perform market research to determine if there is interest from all socio-economic categories: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUB Zone, Small Disadvantaged Business (8a), Woman Owned Small Business, Small Business, or other. The business size standard is for NAICS Code 622110. Synopsis: The Phoenix, AZ Veterans Administration Health Care System Ophthalmic Clinic is soliciting for Outpatient Surgical Care to be performed at the Contractors surgical facility. The Contractor will provide an estimate 4000 ophthalmic surgeries on VA patients per year. SERVICES: The professional service group must provide highly qualified, board certified ophthalmologists and optometrists that have passed parts I, II, and III of the National Board of Examiners in Optometry examination, and completed the Treatment and Management of Ocular Disease (TMOD) exam; with an average of no less than one ophthalmologist per clinic day and necessary number of appropriate providers to meet clinic volume. The credentialing process for these providers must meet the standards of The Joint Commission or equivalent. The group must be capable of furnishing physicians who can provide a full spectrum of ophthalmology services including corneal, medical and surgical retina, glaucoma, i1euro-ophthalmology, oculoplastics and general ophthalmology to VA ophthalmology patients. The care provided must meet the standards of care established by the appropriate boards (American Board of Ophthal mology and the American Board of Optometry). The outpatient clinic care will be provided at the Carl T Hayden VA Medical Center (CTHVAMC) located at 650 E Indian School Rd, Phoenix, Arizona with outpatient surgery being performed at the Contractor's facility. Post-op visits for cataract surgery will be seen at the Contractor's facility Clinic Coordinator: Contractor is responsible for providing a full-time Clinic Coordinator to work on-site at CTHVAMC. Ophthalmic Technicians: The VA Eye Clinic currently has an internal staff consisting of five (5) Health Technicians, one of which primarily functions in diagnostics. The VA may require supplemental coverage from time-to-time. Two (2) contract Ophthalmic Technicians are currently being utilized in the Eye Clinic. Clinics will be categorized by diagnosis, e.g., glaucoma, plastics, retina, general, and teaching clinics. The physician group will establish clinical operating protocols and procedures in compliance with the VA Eye Care Handbook and departmental service agreements to ensure appropriate care is provided in accordance with sound medical practice and guidelines established by the professional organizations: the American Optometric Association and American Academy of Ophthalmology. The VA utilizes an electronic record system called Computerized Patient Record System (CPRS). Contractor's employees will receive training from the VA and will be required to use this electronic system. New appointment calendars and related administrative requirements will be completed in collaboration with designated VA staff. Prescriptions will be written electronically by Contractor or by providers under contract with Contractor and/or via VA prescription pads or forms, as appropriate, e.g., eyeglasses, medications, etc. Contractor's employees must successfully complete credentialing, background check, fingerprinting, and non-employee orientation before starting at the VA. The Administrative Officer, Surgical Service will notify the Contractor when these requirements have been completed. Contractor is required to meet VA performance measures. Current measures require patients to be seen within 14-calendar days and no-shows to be kept fewer than 13%. National performance measures may be adjusted from time-to-time. I n the event that inpatient care is needed, the inpatient care will be provided by the VA. Should an emergent episode present, then the nearest appropriate medical care facility may be used with transfer to the VA when the patient is medically stable. The VA will be responsible for payment for emergent care. General Requirements Providing quality patient eye care to the Veteran in a cost effective, efficient environment must be the ultimate outcome of the Contractor's group. The Contractor must meet The Joint Commission or equivalent (American Academy of Ophthalmology and the American Optometry Association, as appropriate) standards for performance improvement. The results of the performance improvement activities must be available for review. In addition, the Contractor will participate with the VA in all related performance improvement initiatives. Contractor's providers must: oHave an unrestricted license in a US state, territory or US commonwealth; oSpeak English proficiently and possess excellent written communication skills; oProviders must all be capable of efficient use of Computerized Patient Records System; oProvide evidence of demonstrated current clinical competence to perform appropriate Ophthalmic care in clinic and in surgery; oBe a U.S. citizen; oHave a professional degree and education applicable to VA Medical Staff Members; oProvide evidence of current professional liability insurance; oBe board certified or board eligible in Ophthalmology (prefer board ce11ified); oProvide evidence of current privileges (if applicable) oProvide evidence of annual TB testing; oProvide names of most current employer(s) and two peer reference contacts oComplete background check, fingerprinting and non-employee orientation prior to starting. oComplete BLS and maintain current certification, as a privileging requirement for clinically active providers Other necessary personnel such as Health Technicians and Patient Services Assistants (Clerks) will be provided by the VA at levels mutually agreed upon. The VA Eye Clinic currently has an internal staff consisting of five (5) health technicians and three (3) patient services assistants (clerks). The services to be performed by the Contractor, at the Phoenix VA Health Care System, will be performed within the VA policies and procedures and the regulations of the Medical Staff Bylaws. Contractor will establish clinic profiles that will ensure timely initial and follow-up appointments based on annual estimates, in compliance with VA policy to ensure appropriate care is provided in accordance with sound medical practice and guidelines established by the professional organizations: the American Optometric Association and American Academy of Ophthalmology. Initial appointments shall be seen within 14 days and follow-up appointments within seven (7) days or when medically appropriate. Contractor will be required to establish clinical operating protocols and procedures in compliance with the VA Eye Care Hand book and depm1mental service agreements, to ensure appropriate care is provided in accordance with sound medical practice. New appointment calendars and related administrative requirements will be completed in collaboration with designated VA staff. Prescriptions will be written by contractor or by providers under contract with Contractor on electronically or VA prescription pad as appropriat. e.g. eyeglasses, med ications, etc. The Contractor shall not advertise or use any marketing material, logos, trade names, service marks or other material belonging to the VA. VA is a self-insurer. VA employees performing work under a sharing agreement pursuant to 38 U.S.C., Section 8153 are covered by the Federal Tort Claims Act. VA's Office of Regional Counsel processes all administrative claims filed under the Federal Tort claims Act against VA. The Department of Justice, Office of the U.S. Attorney handles litigation in such cases. VA does not provide legal representation for lawsuits fi led against Contractor employees regard less of whether the incident occurred at the VA Medical Center or Contractor's facility. The Contractor shall avoid referral of patients for further evaluation to his/her personal group without clear medical indication. Billing for all services provided by referrals within the group will be reimbursed at the rates negotiated by contract. The Chief of Staff or designee reserves the right to disallow any referrals that lack adequate medical need, or would be better served by a different provider. The normal work schedule shall be Monday through Friday, 7:30 am to 4:00 pm. The term of the contract will be a Base Year plus Four (4) Option Years starting from the date of award for five (5) twelve (12) month periods. Solicitations will be available on or about July 2014. The date is not firm and may change due to events out of our control. This notice is for informational purposes only and is not a request for proposals. The Government is not obligated to issue a solicitation as a result of this notice. No other information is available until issuance of the solicitation. A determination by the Government to compete the proposed procurement based on the responses to this notice is solely within the discretion of the Government.
