Description

Synopsis: Please note that this is NOT a request for quotes or proposals. The Government is seeking information for market research purposes only. The Department of Veterans Affairs, VISN 3, Network Contracting Office 3 (10N3NCO) is currently conducting a market survey for firms that provide Elevator Lockout Key Installation at the James J Peters VA Medical Center in Bronx, New York. Firms must be authorized, qualified, certified, and experienced in providing this type of service. 1.Title of Project CLC Elevator Lockout Key Installation 2.Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified for the above project 3.Background Lock out of Basement for all 3 CLC Elevators for patient safety 4.Performance Period: [Firm-Fixed-Price] The contractor shall complete the work required under this SOW in 3 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. 5.Type of Contract: Firm-Fixed-Price 6.Requirements: Installation of Car Call Lockout key switch in the existing car operating panel for three (3) elevators in the CLC to lock out access to the basement. This security feature will control access to the basement. The new key switch will be a two position on/off key switch and will be wired to render to basement call button inoperable when in the off position. This arrangement will not affect the Fireman's Emergency Operation. One year warranty to be included and a required walk through will be required for all interested parties. Contractor shall furnish all necessary labor, equipment, tools, materials, parts, etc., necessary to complete the installation 7.Place of Performance: U.S. Department of Veterans Affairs James J. Peters VA Medical Center 130 West Kingsbridge Road Bronx, NY 10468 The North American Industry Classification Systems (NAICS) is 561210. Responses to this notice must be submitted in writing via email and must be received not later than Wednesday, January 28, 2015, 5:00pm EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 561210. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov). Responses to this notice must be submitted in writing, and received not later than Wednesday, January 28, 2015, 5:00pm EST.
