Description

The Bath VA Medical Center located at 76 Veterans Avenue, Bath New York has a requirement for a Service maintenance agreement for the following medical imaging equipment: a.Shimadzu Dart Evolution w/Canon 55C (s/n 100327) and one GE Precision 500 R/F Room with Canon Shared Panel b.GE Vivid 7 ultrasound c.GE Lunar Bone Density unit d.GE Vivid I Ultrasound The services required for each of these pieces of equipment are as follows: SHIMADZU DART EVOLUTION W/CANON 55C (S/N 100327) AND ONE GE PRECISION 500 R/F ROOM WITH CANON SHARED PANEL "The contractor shall provide onsite technical support coverage to include travel, parts (excluding Glassware, X-Ray tubes, II's, Monitors, expendable supplies) and labor within 4hr from original call from 8am to 5pm Monday to Friday excluding Federal Holidays, which are defined as: New Years Day, Martin Luther King Day, Presidents Day, memorial Day, Independence day, labor Day, Columbus Day, thanksgiving Day and Christmas Day. "One preventative maintenance and calibration visits during regular business hours between the hours, Monday to Friday, 8:00 a.m. to 5:00 p.m. This visit shall be performed by the eleventh (11th) month of the agreement and coordinated with the Contract Officer's Representative. All calibrations shall be documented at the time of the service and left with the Contracting Officer's Representative upon completion of the service call. "Software updates for the Shimadzu/Canon system shall be provided to the VA at no additional cost as soon as they are available to the public. GE VIVID 7 ULTRASOUND "Coverage includes the following devices: B&W Printer, Color Printer, DVD, DVR, General Probes: 2, Line Printer and VCR "Coverage excludes the following devices: Pedoff Probe, Specialty Probes, System Integrated UPS (Excludes Floor Models), TEE Probes "Labor and travel coverage for on-site service from 8:00 am - 5:00 pm, Monday - Friday excluding Federal holidays, which are defined as New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. "Contractor shall respond to calls in one hour and be on-site within 6 hours for emergency repairs. " One Quality Assurance/Equipment Quality Control QA/EQC service to include an Image Quality Control (IQC) service as per manufacturer's specifications to be completed by the eleventh months of the task order. The QA/EQC service shall be coordinated beforehand with the Contracting officer's Representative (COR). " Parts coverage, excluding consumable items "24 hour a day/ 7 days a week Unlimited Remote Service access. "24 hour a day/ 7 days a week access to TIP Answer Line. " 97% Uptime Guarantee, per the terms and conditions of the Uptime Guarantee. "Service Management Reports upon request by the Government. LUNAR BONE DENSITY UNIT "Coverage includes the following devices: Tube Coverage, Detector Coverage, External Hard Drive, Host PC, Monitor, Printers "Comprehensive parts and labor support for one on-site service call from 8:00 am - 5:00 pm, Monday - Friday excluding Federal holidays, which are defined as New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. "Contractor shall respond to calls within one hour and be on-site within 72 hours for emergency repairs. "One planned preventative maintenance service as per manufacturer's specifications to be completed by the eleventh months of the task order. The planned preventative maintenance service shall be coordinated beforehand with the COR. "24 hour a day/ 7 days a week Unlimited access to technical phone support. "Contractor shall provide any software and Safety &Quality updates at no additional charge VIVID I ULTRASOUND "Coverage includes the following devices: B&W Printer, Color Printer, DVD, DVR, General Probes: 1, Line Printer, Pedoff Probe: 1, VCR "Coverage excludes the following devices: Carts, Intracardiac Echo (ICE), Including Connector and S/W; Excluding Catheter, TEE Probes "Labor and travel coverage from 8:00 am - 5:00 pm, Monday - Friday excluding Federal holidays, which are defined as New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. "Contractor shall respond to calls within one hour and be on-site within 6 hours for emergency repairs. "24 hour a day/ 7 days a week Unlimited access to technical phone support. "One EQC service and two IQC services as per manufacturer's specifications to be completed by the eleventh months of the task order. The EQC/IQC services shall be coordinated beforehand with the COR. "95% Uptime Guarantee " Accidental damage probe replacement (Unlimited). "A.1.2.3.3.7 Contractor shall provide a loaner hand held probes to the Bath VAMC while scheduled and unscheduled repairs or maintenance of the VA's covered handheld probe occurs. The cost of shipping and handling is included in the price of this service agreement. The NAICS Code is 811219. The expected cost range for this firm fixed price contract to include four option years is between $250,000.00 and $500,000.00. This sources sought is to determine whether there is sufficient interest to warrant either an open market or a small business set aside competition for this procurement. All responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the Canandaigua VAMC. Please note at least 50 percent of the cost of personnel for contract performance shall be spent for employees of the concern for a proposal to be considered for a small business set-aside.
