- SCA Wage Determination Vendor Forms Terms & Conditions Contains Scope of Work This is a subcontracting opportunity with Management & Training Corporation (MTC), operator of the Hawaii Job Corps Center (HJCC) for the U.S. Department of Labor. MTC is requesting bids for the stripping and re-waxing of HJCC's floors. Please provide a price quote on the attached bid sheet for the following Scope of Work: 1) Dining Hall a. Scrub floor and apply two (2) coats of wax b. Est. 3465 sq. ft. 2) Recreation Building a. Strip and apply four (4) coats of wax b. Multiple sections (est. 5644 sq. ft. total) i. Recreation Hall, est. 3240 sq. ft. ii. Arts & Craft Room, est. 804 sq. ft. iii. Learning Resource Center Room, est. 580 sq. ft. iv. Hallway, est. 1020 sq. ft. 3) Health & Wellness Clinic a. Strip and apply four (4) coats of wax b. Est. 1450 sq. ft. 4) Security Office a. Strip and apply four (4) coats of wax b. Est. 280 sq. ft. 5) HJCC will move all furniture as necessary 6) Bidders are required to verify all measurements and areas to be serviced prior to bidding 7) Areas to be serviced will be scheduled at HJCC's discretion and might not be concurrent. Services to the Health & Wellness Clinic and the Security Office are tentative and may not be performed. Services are to be performed at: Hawaii Job Corps Center 41-467 Hihimanu Street Waimanalo, HI 96795 To schedule a site visit, please contact Marc Morikami (808-259-3203, morikami.marc@jobcorps.org) BIDS MUST BE RECEIVED NO LATER THAN 4:00 PM (HAWAII ST) ON DECEMBER 5, 2014 VIA EMAIL (PREFERRED) OR DELIVERED TO THE HAWAII JOB CORPS CENTER. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final. All bidders are subject to the terms and conditions. Bid Submission: A complete bid submission shall include: - Bid sheet with pricing - Signed Acceptance of Terms & Conditions form - Completed Self-Certification form - W-9 MTC Terms and Conditions: Management & Training Corporation (MTC) Terms and Conditions are included with this RFQ, and can be found at www.mtctrains.com. It is the Contractor's responsibility to verify MTC's current Terms and Conditions at the time services and/or goods are provided. North American Industry Classification System (NAICS): Bidder must self-certify under the following NAICS code (NAICS codes can be found at http://www.census.gov/eos/www/naics/): 561720 (Janitorial Services) or 238330 (Flooring Contractors) Service Contract Act (SCA): This service is subject to the provisions of the Service Contract Act (SCA) of 1965, as amended (see FAR 52.222-41). Subcontractors must comply with WD# 2005-2153 (Rev 15) dated 06/13/2011. If this subcontract includes option year(s) and/or an extension, the subcontractor shall be notified if the referenced WD# is revised by the Department of Labor. Wage determinations can be found at http://www.wdol.gov. The subcontractor is required to post a notice of the compensation required (including, for service contracts, any applicable wage determination) in a prominent and accessible location at the worksite where it may be seen by all employees performing on the contract. Federal Funding and Accountability Act of 2006 (FFATA): For organizations, projects with a value of $25,000 or more (and any modifications to a project that change previously reported data) may be subject to the provisions of FFATA, per FAR 52.204-10 Reporting Executive Compensation and First-Tier Subcontract Awards. Reported information will be made public via the FFATA Sub-award Reporting System (FSRS.gov). For applicable subcontracts, this information must be provided to (MTC/Hawaii Job Corps) prior to the final award of the subcontract using the enclosed FAR 52.204-10 First-Tier Subcontractor Information form. The vendor must acquire a DUNS number to fulfill this requirement.
