Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA262-15-Q-xxxx and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-77. This is a small business set-aside and the associated NAICS code is 238220 and small business size standard is $15.0M. The requirement is a small business set aside. See attached statement of work for description and requirements. Place of performance is VA Long Beach Healthcare System, 5901 East 7th Street, Long Beach, CA 90822. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-77. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: https://www.acquisition.gov/Far/. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors -Commercial Items, FAR 52.212-3, Offeror Representations and Certifications-Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications or confirm that the annual representations and certifications was completed electronically via the SAM website accessed through http://www.acquisition.gov; FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.219-6, 52.219-6, 52.219-13. 52.219-28, 52.222-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-13, 52.232-34. The following VAAR Clauses are applicable 852.203-70, 852.211-70, 852.232-72, 852.246-70, and 852.246-71. Evaluation and Award This is a competitive Request for Quotations (RFQ) that will be evaluated under FAR Parts 12 and 13. The Government intends to make one (1) award from this RFQ. The contractor shall possess medical gas certifications for testing medical gas piping distribution systems. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, Lowest Price Technically Acceptable. Offers should be submitted electronically by the 3pm (PST) on 10/23/2014 to layton.matthews@va.gov. Offerors shall indicate that they possess the required medical gas certifications for testing medical gas piping distribution systems. In addition to completing the Price/Cost Schedule, the offeror shall provide technical descriptions in sufficient detail to allow the Government to ascertain the Offerors capability to meet the requirement. It will not be the Government's responsibility to obtain any technical information not provided. An Offeror's initial quote should contain the best terms from a price and technical standpoint. All questions and concerns regarding this solicitation shall be directed to the Contracting Specialist. Includes: Supply Source Control Devices (Main and Zone Valves) Distribution Outlets/Inlets System Monitoring (Warning Alarm System) Option Year Four (4) Period of Performance: 11/1/2018 - 10/31/2019 BASE PLUS OPTION YEARS TOTAL: ___________ STATEMENT OF WORK Medical Gas and Vacuum System Annual Preventive Maintenance Inspection and Testing Report The contractor shall provide annual preventive maintenance inspection and testing reports of medical gas and vacuum system this requirement is needed to be performed every October during normal business hours of operation at VA Long Beach Healthcare System. The evaluation and inspection requirements shall be in compliance with National Fire Protection Association (NFPA) 99 and ASSE Series 6000. The inspection and testing reports are mandated by The Joint Commission. The Medical Gas Piping Distribution System is comprised of four major components. For optimum functional capacity, the contractor shall provide preventive maintenance inspection as outlined below: 1.Supply Source oProper System Configuration oSystem Leakage oGrade "D" Compliance (Medical Air Systems) 2.Control Devices (Main and Zone Valves) oAppropriate Labelling for Area, Gas, Function oCheck for Leaks oCheck for Distribution Capability 3.Distribution Outlets/Inlets oAppropriate Labelling oGas Inspection oPressure Capability oCross Connection Testing Leakage 4.System Monitoring (Warning Alarm System) oAppropriate Labelling oSystem Monitoring (Alarm Monitor Panels oAudible oVisual REPORTING The contractor shall also provide Inspection and Testing Reports and all required documents to the Mechanical Pipe Shop Supervisor. In the event that preventive maintenance inspection cannot be completed by end of October for each period of performance, the contractor shall provide a written letter to the Mechanical Pipe Shop Supervisor stating the reason and propose test date.
