- Electronic Bid Schedule dated 14 Oct 2014 PWS dated 14 Oct 2014 Amendment 0004 The purpose of this amendment is: 1) To re-open and extend the Due Date and Time for receipt of offers in Box 8 of the original RFP until Friday, 31 Oct 2014 at 12:00 noon (CST). 2) To incorporate revised version of the PWS dated 14 Oct 2014. 3) To incorporate the revised version of the Electronic Bid Schedule dated 14 Oct 2014. INSTRUCTIONS TO OFFERORS: For all eligible offerors interested in submitting a response to this RFP, please follow instructions in addendum to FAR 52.212-1 and submit the response no later than the date and time established on Box 14 of this amendment. All responses and questions to this solicitation should be addressed to the new POC listed at the end of this notice revision. For proposals submitted prior to re-opening of the solicitation : As a minimum, in order to be eligible for award, offerors must submit a signed copy of Amendment 0004 if their previously submitted prices and terms and conditions remain unchanged. In the event that the offeror needs to provide any revisions to their previously submitted proposal, the contractor shall provide a complete revised proposal in accordance with the addendum to FAR 52.212-1 entitled "Instructions to Offerors - Commercial Items" along with a signed copy of the amendment. Replacement portions of previously submitted proposals will not be considered nor evaluated; therefore, such submissions will be deemed non-conforming to the terms of the solicitation and thereby not eligible for award. Contractors who previously submitted an offer are highly encouraged to confirm that subject response was received. Site Visit: No new site visit will be scheduled. All questions regarding the terms and conditions in response to this solicitation must be submitted in writing no later than 12:00 (noon) CST on Friday, 24 Oct 2014 to the following POC: Andrew S.
