- Extended Warranty Certificate Alert Plus 32 Port System T his is a combined synopsis/solicitation for commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quotes (RFQ) number is title F5N1SS4231A001. This solicitation is being issued as 100% small business set aside under the North American Industrial Classification Standard code 334210 with employees less than 1,000. Products to be delivered to Westover ARB Chicopee, MA 01022. This request for quotes is for an Annual Remote Support Agreement for an 8-32 port chassis. This Support Agreement will run for a total of one year from the contract award date. Contractor will perform remote diagnostics and cover advance replacement if required; equipment serial numbers are 11837, 11838, 11839, and 11840. All products and services shall conform to the terms, standards and conditions of all applicable laws, regulations, standards, and best commercial practices with the focus on safety and customer service. Quotes will be evaluated on a Lowest Price Technically Acceptable (LPTA) basis. Attached is the Extended warranty certificate on the subject Alert Plus, 32 Port System. To receive an award of contract the offeror must be registered in System for Award Management (SAM) database which can be accessed at the website https://www.sam.gov/portal/public/SAM. Offers shall be submitted via email NO LATER THAN 1000 AM EST, 13 October 2014 to kimberly.mcgee.2@us.af.mil.
