Description
I. SYNOPSIS: 1. The contractor shall furnish all labor, equipment, transportation and fuel to transport and dispose of solid waste generated at Fort Snelling National Cemetery, Minneapolis, MN 55450 in accordance with Environmental Protection Agency (EPA), Executive Order 13423, and Department Of Transportation regulatory requirements. The requirement is for a base year and four (4) option years, if exercised. II. RESPOND TO: 1. All RESPONSES shall be submitted via email to kenneth.dougherty@va.gov III. Background / Scope of Work: 1. Removal of Waste Material (General), NAICS 562111 a)The contractor shall furnish two (2) 30 yard OT on-call containers to be kept on-site and hauled as needed - approximately once per month per container. The material in the containers will be general MSW and old/dead floral arrangements. b)The contractor shall furnish one (1) 10 yard OT on-call container to be kept on-site and hauled as needed - approximately once per month. The material in this container will be broken up granite/marble/concrete (largely composed of headstones/markers destroyed by cemetery personnel). c)The contractor shall furnish seven (7) 30 yard OT on-call containers for cemetery winter cleanup operations to be conducted the first weekend in February. The containers shall be delivered on the Friday preceding the first weekend in February, kept on-site over the entire weekend, and removed/hauled the Monday following the first weekend in February. The material in the containers will be primarily old/dead floral arrangements and wreathes. d)The contractor shall identify to the Contracting Officer, a project manager and alternate who will act as a point of contact with regard to this contract. e)The contractor shall empty containers within one (1) working day of the request made to empty the containers. If containers cannot be emptied within the required timeframe, the contractor shall provide the following in writing to the Contracting Officer and COR: (i) the reason for the delay, and (ii) the anticipated date when the container will be shipped and emptied. This written notification shall be required within 24 hours of finding the conditions causing the delay. f)The contractor shall schedule shipment dates and times with the COR. Shipments shall not be scheduled on Saturdays, Sundays, federal holidays, or any other day when the Fort Snelling National Cemetery is closed. Shipments shall be scheduled so that all loading operations can be completed before 4:00 p.m. on the day that the shipment occurs. 2. All work will be accomplished during normal cemetery hours of operation (8:00 AM- 4:30 PM). To schedule work outside of normal cemetery hours, the vendor must coordinate the date and time with the Assistant Director of Fort Snelling National Cemetery. 3.Contractor's equipment shall be in good working condition. 4.Contractor shall send invoices to proper address for payment and bill only for services done. 5.All Contractor personnel working on site shall be experienced and qualified to perform the required work. The Contractor employees shall conduct themselves in a decent, orderly and business-like manner at all times while on Cemetery property. Failure to do so may result in termination of the contract. 6.The Contractor shall not assign, transfer or sublet this contract to any other entity without the express written approval of the Contracting Officer. 7.The Contractor shall comply with all Federal, State and Local laws, codes, rules and regulations relating to the performance of the work and the protection of property and the public. IV. SUBMISSION REQUIREMENTS / QUESTIONS: 1. Please advise if you ever provided the above requirement? Please briefly describe your experience. 2. Please advise if you are able to provide such requirement in the near future at the above national cemetery. 3. Include your name, business address, name of company representative, contact information, DUNS Number, Email, PHONE # AND completed FAR Provision 52.212-3 "Certifications and Representations", business qualification for socio-economic categories (e.g., Small Business, Small Disadvantaged Business, Service Disabled-Veteran Owned Small Business, Women-owned Small Businesses, HUBZone Small Businesses, etc.). If no business qualification for socio-economic categories applicable, please advise if you are a large business. V. RESPOND TO: 1. All RESPONSES shall be submitted via email to kenneth.dougherty@va.gov The formal closing date for this RFI and for the submission of responses is October 03, 2014 at 1:00pm EDT.
