- SINGLE SOURCE (APPLICABLE TO PURCHASES UNDER SAT $3,000 - $150,000) 1.Contracting Activity: NCO 12, GREAT LAKES ACQUISITION CENTER, 115 SOUTH 84TH STREET, SUITE 101 MILWAUKEE, WI 53214 2.Purchase Request Number: 578-14-4-116-0135 3.Authority for Action: 1 FAR 13.106-1(b), Soliciting from a Single Source 4.Describe the supply or service to be acquired and the total estimated dollar value: The Hines VAMC has a request for a new firm fixed price contract to perform consultation on the study Mindfulness Based Stress Reduction for Women at Risk for Cardiovascular Disease. The total cost is for $90,604.00. one (1) base year and three (3) option years. Grant-Project #NRI 12-413. 5.List the proposed contractor (name, contact, and phone number) and describe the unique knowledge or capabilities of the proposed contractor: LJanusek Consultants Linda Janusek 798-246-7623 652 Courtland Cir. Western Springs, IL 60558 LJanusek Consulting has specific expertise in the area of randomized clinical trials evaluating health outcomes of Mindfulness Based Stress Reduction (MBSR) as well as in analysis and interpretation of proinflammatory cytokines, cortisol, and DNA methylation related to cumulative life stress. Dr. Saban specifically requests LJanusek Consultants for Grant Project #NRI 12-413. 6.Describe your market research efforts and efforts to ensure that offers are solicited from as many potential sources as is practicable: Market research found 2 vendors that provide MBSR consultation services within the Chicago area. LJanusek Consultants and Rush University Medical Center provides Mindfulness Based Stress Reduction consultation services. Research did not reveal any additional vendors that supply such research. 7.State why you feel that the anticipated cost to the Government will be fair and reasonable: IAW FAR 13.106-3, price reasonableness may be based on a comparison with previous purchases for similar items or services. Mindfulness Based Stress Reduction services were previously obtained for Hines VA Medical Center through contract # VA69D-13-P-5093 for $22,651.00 per year which is the same cost as the current procurement. The CO determines the anticipated cost for this procurement to be fair and reasonable.
