- Tohatchi Health Center (THC) Site Plan Performance Work Statement (PWS) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is RFQ No. 245-GIMC-15-005. The solicitation is issued as request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. This RFQ is for dental vacuum pump replacement for the Tohatchi Health Center, Tohatchi, New Mexico of the Indian Health Service (IHS). The tasks to be performed are cited in the attached Performance Work Statement (PWS). The associated NAICS code 238290, which has a small business size standard of 14.0 million dollars, is applicable. A firm fixed-price contract is anticipated to result from award of this solicitation. The proposed required services are for November 14, 2014 - December 19, 2014. To be considered for award, please submit the following required documents: -Pricing Quote to include the following contract line item numbers (CLINS): 1. Service/Supplies for complete replacement of vacuum pump* 2. Total Equipment - Vacuum Pump 3. Gross Receipt tax on labor only *Line Item 1 shall be all-inclusive to provide for all labor, fringe benefits, material, supplies, transportation, per diem, supervision and all other necessary expenses to provide required services/supplies. -Company capability statement including past performance history of the last 3 years, experience, and qualifications. Also, include similar projects to the one proposed. -Quality Assurance plan for the proposed work. We have attached a THC site plan. We will facilitate site visits and will be accommodated by our Facilities manager, Ali Ali, 505-722-1558, email: ali.ali@ihs.gov. 52.237-1 Site Visit Offerors or quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award. ***A site visit will be scheduled for Thursday, October 23 at 10:00AM at Tohatchi Health Center, 07 Choosgai Drive, Tohatchi, NM. If you plan on attending you will need to email points of contact (Adam Reinbolt & Ali Ali) by 4:30PM MST on October 17, 2014. We will be meeting in the lobby at the front entrance of the Tohatchi Health Center.*** Please submit a quote, along with all supporting documents by November 7, 2014, 4:00pm MST. The Government reserves the right to issue a single award to the Schedule contractor whose quote represents the best value as defined by FAR 2.101 and results in the lowest-cost alternative for the Government. In determining best value, in addition to price, the following factors will be considered: Management Approach and Technical Capabilities, Personnel Qualifications, Organizational Experience and Past Performance. The socio-economic status of an offer may also be considered should more than one quote represent the best value and lowest-cost alternative.
