SPECIFICATION SHEET Department of Veterans Affairs Furniture for Community Living Center (CLC) Houston, Texas Introduction The Michael E. DeBakey VA Medical Center in Houston is seeking for Community Living Center (CLC) patient rooms, new bedside cabinets and storage units. These units are custom to the needs of CLC patients and must be safe in design and constructed of quality and environmentally safe materials. Information The current bedside cabinets and storage units are in poor unacceptable condition and can no longer be repaired and do not meet the needs of our veteran CLC patients.. The standard stock design does not lend itself to functional use for the patient population. These custom designs have been reviewed and approved by the Service Chief, designated staff and veteran patients in the CLC service. Design and finish materials meets infectious control and joint commission standards. Furniture requested meet ergonomic standards and ADA requirements. All products shall be Greenguard Indoor Air quality certified and Greenguard Children and Schools certified. Registration of firm shall be confirmed on the web site for verification. http://prodcutguide.uelenvironment.com/QuickSearch.aspx. A requirement of this VA order is that it shall be awarded to a qualified Service Disabled Veteran Owned Small Business (SDVOSB) CLIN 00001 Product Specifications "QTY 127 Custom bedside table cabinets " Nemschoff model C7101405_A OR EQUAL "Foxxman: Special Prague Series "Custom unit to meet specific needs of CLC patient population "Finish: Durawrap OR EQUAL from Vendors standard selection: oWY- Medium cherry oSubstitution shall be presented to VA medical center for approval. Actual sample only. Paper submittal will not be acceptable. "Dimensions are to be exact: 24 7/16"w x 37"h x 24"d oAllowances are not acceptable due patient population served "Actual product literature regarding "equal item" shall be included for items to be considered as "equal" to brand name. A "Direct copy of text from this Statement of Work" shall not be acceptable. " Offerer shall submit in package the exact make, model number, and description of items. A "Direct copy of text from this Statement of Work" shall not be acceptable. "Offerer shall be an authorized dealer of the manufacture's product to be considered a qualified submittal. Documentation shall be provided by furniture manufacturer that offerer is on record as an authorized dealer. "Required engineer shop drawing of any "equal" product from offerer shall be submitted as part of the package "Finish shall be Durawrap or equal; a thermal fused laminate with is wrapped around the substrate to prevent moisture gain. ADDITIONAL PRODUCT SPECIFICATION INFORMATION Brand name -Nemschoff or Equal Bedside tables for Community Living Center (CLC) oVertical front: outside edges of drawers and doors shall have edges profile as Prague II, modified oBottom front of cabinet shall be finished with a front base rail to match door and drawer fronts. oSide and back panels to be continuous to the bottom edge of front rail to create a leg and bracket foot appearance at front of cabinet. oCabinet shall have 4 3" casters rubber wheel, twin with two front locking toe tabs. oInterior cabinet shall be comprised of three wood box drawers. oTop drawer shall have digital re-programmable lock. oDrawer construction shall be: four sided multi ply box, dove tail joinery all four corners. Wood bottom dado-ed and glued in place. Drawer front shall attach separately to drawer head. oEntire box shall be sanded and sealed to maintain patient safety and clean-ability. oSuspension guide system shall be heavy duty suspension guides mounted at bottom of wood side panel to handle 100 pound weight capacity. oDrawer suspensions shall have positive in-stop in two top drawers. oCase components shall be constructed using balanced ply construction utilizing 45 # medium density fiberboard substrate permanently fused to laminate faces on both sides. Fusion shall take place under controlled environment using heat and pressure and shall meet NEMA standards for vertical laminate. Edges of panels shall be fully sealed to prevent moisture regain. oAssembly of all components shall allow for full field replaceability. Mechanical fasteners shall be comprised of a two component system using 34.7 mm hardened steel dowels with a 25mm screw thread and depth stop washer to provide 350 pounds of shear strength and resistance to pull out. Full field replaceability; documentation of this requirement shall be on furniture web site; link shall be provided for confirmation. oReceiving member of fastener shall be 25mm x 12mm to be pressed into the panels and held in place by serrations. oTop of case shall have solid wood perimeter surround 7/16" high to prevent objects from falling off of cabinet. oFinish shall be Durawrap or equal; a thermal fused laminate with is wrapped around the substrate to prevent moisture gain. This process seals edges without visible seams. "Equal" offerer of product shall provide documentation of no visible seams. Link to information shall be provided. A physical sample provided upon award for final selection. oAll hardware and joinery mechanisms shall utilize tamper resistant screws. oSeller shall warrant the products it manufactures against defects in manufacture and materials. Preferable LIFETIME from date of shipment to original buyer; provided that the product is in use for its intended purpose. oWarranty shall be void in case of damage through negligence, misuse or abuse of product and damage in transit or as a result of accidents. oPreferable LIFETIME Warranty from date of shipment for all structural and all operational parts, finishes and labor. oActual product literature regarding "equal item" shall be included for items to be considered as "equal" to brand name. DeBakey Veterans Affairs Medical Center, 2002 Holcombe Blvd, Bldg #100, Houston, TX 77030. Furniture will not be able to be assembled within patient rooms. Assembly will need to be completed prior to delivery to patient rooms. Delivery and installation shall not interfere with hospital staff or patients. Delivery and installation scheduling will need to be coordinated with Contracting Officer Representative. Preferred form of communications is email. Vendor shall provide a 5 days' notice prior to delivery to the Contracting Officer Representative for coordination of delivery and installation. A Veterans Affairs contractor will be removing the existing government furniture from site. A Field Supervisor must be on site during delivery, removals of existing furniture, and installs & shall have a minimum of 18 years of experience of size and scope of this requirement is preferred. Veteran Affairs employees shall not off load any products. Delivery shall be completed during normal work hours, Monday through Friday, 8:00am - 4:30pm local time, excluding for Federal Holiday. Veterans Affairs Medical Center warehouse will not be able to store any product prior to installation, or if unable to accept delivery/installation. Quantity at each delivery shall be 10 Armoires and 10 beside cabinets. Each delivery shall be 3-5 days apart from the previous installation. See Delivery Schedule with projected installation dates, actual installation date are flexible, and must be coordinated with Contracting Officer Representative. GREENGUARD All products shall be Greenguard Indoor Air quality certified and Greenguard Children and Schools certified. GENERAL INFORMATION and REQUIREMENTS oActual product literature regarding "equal item" shall be included for items to be considered as "equal" to brand name. Direct copy of text from this Statement of Work shall not be acceptable. oHard copies of Brochures for proposed furniture illustrating dimensions, pictures and materials are required with submittal if *Equal* product is to be considered. o Please provide a 2 and 3-D drawings depicting typical in package oOfferer shall be an authorized dealer of "equal" product to be considered a qualified submittal. Documentation shall be provided by furniture manufacturer that offerer is on record as an authorized dealer. oRequired engineer shop drawing of "equal" product from offerer shall be submitted as part of the package oActual samples of finishes shall be presented to VA medical center for Interior Designer/Contracting Officer Representative to approve for final selection upon award. Color and finishes shall not be selected from pictures upon contract award. oOne assembled sample of each bedside cabinets and the Armoire shall be manufactured if "equal" offerer is awarded the contract, and samples shall delivered to VA Medical Center to the Contracting Officer Representative for confirmation of product prior to production of contracted quantities. oPreferable LIFETIME Warranty from date of shipment for all structural and all operational parts, finishes and labor. Warranty shall be documented on furniture website; link shall be provided for confirmation oFurniture shall meet demands of a 24 /7 hour use; product specification use shall be stated in offer oFurniture shall include hardware to install oFurniture components shall all be field replaceable; documentation of this requirement shall be on furniture web site; link shall be provided for confirmation. oFurniture must meet ADA guidelines; link shall be provided for confirmation oCOR/VA designer will select finishes with awarded vendor oFurniture order needs to be coordinated with COTR/VA interior designer oInstall will need to be coordinated with COTR/VA interior designer oThe installation shall occur Monday-Friday between the hours of 8:00 AM 4:30 PM, except for Federal Holidays. oInstall time and design/specifications must be included in bid quote but be listed as separate line items. oRemoval and disposal of existing products shall be included in bid.
