- OCSQ39320140170SOWTEC REQUEST FOR QUOTE OCSQ-393-2014-0170 This is Request for Quotation prepared in accordance with FAR Part 13, as supplemental with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION: A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services (CMS) intends to award on a competitive basis, under the Simplified Acquisition Procedures in accordance with FAR Part 13 (Dollar Threshold of $150,000), a firm fixed price award for "Nursing Home Regulatory Policy and Processes Consultant." The Statement of Work and Technical Evaluation Criteria are attached. Responses shall be no more than 10 pages and shall include the following: 1) Technical Understanding and Approach, 2) Personnel, 3) Management, and 4) Price - fully loaded labor rate and anticipated hours. The award will be made on the basis of lowest price technically acceptable meeting or exceeding the acceptability standards for non-cost factors. All responsible sources must be registered @ www.sam.gov. The period of performance is one base year September 23, 2014 - September 22, 2015 with 1 option year. The North American Industry Classification Code (NAICS) is 541990. Responses must be sent electronically in MS Word Format to Lucille.Lee@cms.hhs.gov no later than 11:00am on August 13, 2014. ALL QUESTIONS SHOULD BE SUBMITTED ELECTRONICALLY BY 11:00AM on August 6, 2014. QUESTIONS WILL NOT BE ANSWERED BY TELEPHONE. All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated.
