- ACH Vendor Form RFQ 2014-Q-16723 The Centers for Disease Control and Prevention (CDC), National Institute of Occupational Safety and Health has a requirement for "360 Mining Classroom Training Simulator and Continuous Miner Module." This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes/proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) as number 2014-Q-16723 using FAR Subpart 13.5 Simplified Acquisition Procedures. Any incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76, effective Jul. 25, 2014. This RFQ is 100% set-aside for small business. The North American Industrial Classification System Code is: 541519 with a small business size standard of: $25.5 million. Line Item 0001 - Classroom Simulator. Period of performance: approximately 24 months. See attached document(s) for Statement of Work, Addendum, Clauses, detailed solicitation, and other additional requirements/items. Electronic quotations will be accepted. All prospective offerors interested in submitting a quote must include company name, mailing address, point of contact, phone number, e-mail address, business status, DUNS, and information as required contained in this announcement.
