- This is the scope of work & bid information sheet. This is the OJCA food service items bid sheet (FORM#'BID-1'). This is a blank IRS form W9. This is ETR's terms & conditions and applicable FAR clauses. This is ETR's Vendor Acknowledgement Form. "THIS IS A SUBCONTRACTING OPPORTUNITY" *Award is/may be contingent upon ETR Corporate & U.S. Department of Labor Regional Approval. *Note to this subcontract opportunity solicitation *Funding out notification to all interested parties Either party, upon thirty (30) days written notice to the other, may terminate any agreement. A notice shall be effective on the same date as duly posted in the United States Certified Mail, and shall be addressed to the subcontractor. In the event the prime contractor's contract with the U.S. Department of Labor is terminated by that agency for any reason, the effective date of that termination will also apply to the goods or services subcontractor, unless terminated sooner by the prime contractor as stated above. Vendors are needed to provide and deliver (as specified in the FBO advertisement) Food Service Supplies/Juice for the 291 +/- students of the Oneonta Job Corps Academy.
