Description
NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention (CDC), National Institute for Occupational Safety and Health (NIOSH), intends to negotiate a sole source purchase order to Geographic Information Services, Inc., 2100 Riverchase Center, Ste 105, Birmingham, AL 35244 that is supported by a Sole Source Justification and Approval. The Centers for Disease Control and Prevention (CDC), National Institute for Occupational Safety and Health (NIOSH), has a requirement for Upgrade of Interactive Atlas of Heart Disease and Stroke Web Application. Draft Scope of Work The Division for Heart Disease and Stroke Prevention (DHDSP) is committed to documenting the burden of heart disease and stroke. In addition to disparities in the burden of cardiovascular diseases and their risk factors by race, sex, and gender, geographic disparities have been identified. Identifying the places that bear the greatest burden of cardiovascular disease allows for the generation of hypotheses and is the first step to tailoring resources and interventions to the needs of specific geographic areas. In 2012, DHDSP launched our web application to interactively display heart disease, stroke, and health services data at the county level (http:// nccd.cdc.gov/DHDSPAtlas). This site is part of CDC's National Cardiovascular Disease Surveillance System and addresses the goals of monitoring trends and patterns, setting research priorities, monitoring quality of care and patient outcomes, identifying underserved populations, and planning the placement of services. Users of these maps include: DHDSP staff, state and local health departments, healthcare providers, other public health professionals, academics, and the general public. The Atlas is also an important tool for guiding and supporting activities that address the Million Hearts Campaign. As DHDSP and our users continue to visit the site, we have identified additional functionality that would add value and improve the functionality of the application. C.2 PROJECT OBJECTIVE The objective of this work is to provide an update to the Interactive Atlas of Heart Disease and Stroke to improve and augment several of the website's functions and to enable it to function in the new CDC computing environment and web-based template. If funding is available, we anticipate additional enhancements to the site to be completed in an option period. Period of Performance: Base: Six (6) months Option 1: Six (6) months ********** This contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source and is supported by a sole source justification and approval under the authority of FAR 13.106-1(b) (Simplified Acquisition Procedures (SAP)) and 10 U.S.C. 2304(c)(1). Responsible sources that believe they possess the expertise and capabilities identified above are hereby afforded the opportunity to submit to the Contracting Officer within 15 days from the posting date of this notice, their written capabilities statement and pricing information in the format they choose, not to exceed 10 pages. All vendors must be registered in the System for Award Management (SAM) at www.sam.gov, prior to an award of a federal contract. This is not a Request for Proposals; however, the Government will review any/all product, capabilities, and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), a purchase order will be issued to Geographic Information Services, Inc. as the only source.
