- Amendment II - The purpose of this Amendment No. 0002 dated 7/24/2014 is to correct the description provided in Amendment I for the new CLINs 0006, 1006, 2006, 3006, and 4006. Delete: For the new CLINs 0006, 1006, 2006, 3006, and 4006, provide your loaded hourly rates and leave the unit price and the extended amount as tiny_mce_marker.00 since emergency repairs are currently unknown. Insert: For the new CLINs 0006, 1006, 2006, 3006, and 4006, provide your loaded hourly rates and leave the unit price and the extended amount as $0.00 since emergency repairs are currently unknown. Amendment I - The purpose of this Amendment No. 0001 dated 7/23/2014 is to allow for emergency response and repairs to be funded by the resulting purchase order. All text that stated that no emergencies repairs and parts will be funded by this order has been removed. The RFQ title, RFQ CLINS 0005, 1005, 2005, 3005, and 4005, and the SOW text have been updated. Emergency response and repairs have been added to the Combined Synopsis/Solicitation and the SF18 as unfunded Optional CLINs 0006, 1006, 2006, 3006, and 4006. A revised ATTACHMENT I Combined Synopsis/Solicitation and a revised ATTACHMENT III SF 18 are now attached and replace previously posted documents. Please use the revised attachments to prepare your quote. ATTACHMENT II is not being revised. Your pricing information should be placed on the provided revised SF18 not the Combined Synopsis/Solicitation. For CLINs 0005, 1005, 2005, 3005 and 4005, provide the price for one inspection and report for each performance period and include this same price for each period to the unit price and the extended amount for that period. For the new CLINs 0006, 1006, 2006, 3006, and 4006, provide your loaded hourly rates and leave the unit price and the extended amount as tiny_mce_marker.00 since emergency repairs are currently unknown. Intial Posting The combined synopsis/solicitation is included as Attachment I. The Department of Labor Wage Determination is included as Attachment II. Standard Form 18 is included as Attachment III.
