- WD 2005-2069 Rev 13 WD 2005-2133 WD 2005-2419 Rev 17 PWS Solicitation The Contractor shall provide a full range of psychological health case facilitation, referral services, and support through outreach activities to AFRC Reservists and their families. These services shall be regionalized. The contract services shall include, but are not limited to, case facilitation (not clinical case management); command consultation; planning and coordinating regional psychological health referral services; providing psychological health information in support of Yellow Ribbon Reintegration Program events; and administrative support to document services provided, report statistics, and provide PHAP customer feedback taken from satisfaction surveys. Contractor personnel are not authorized to counsel, diagnose, or treat any person requesting assistance. The places of performance are Wright Paterson AFB, OH, Dobbins ARB, GA, and Travis AFB, CA. The period of performance for this contract is anticipated to be a one-year basic contract period, four-annual option periods and a six-month option. The solicitation method to be utilized is for a Request For Proposal (RFP). This acquisition is anticipated to utilize Lowest Price Technically Acceptable source selection prodedures. This pricing arrangement will be Firm-Fixed-Price with a Cost reimbursable travel CLIN. The NAICS code is 541611 and a size standard of $14 million in annual revenue. The solicitation will be issued as a small business set-a-side. All offerors must be registered in System for Award Management (SAM).
